Imaging and Treatment of Complications of Abdominal and Pelvic Mesh Repair

Abstract: Surgical mesh is used most frequently for tension-free repair of abdominal wall hernias in adults, because the rate of hernia recurrence is lower with mesh than with primary soft-tissue repair. Since the introduction of polypropylene mesh in the middle of the 20th century, many mesh materials and configurations for specific surgical procedures have been developed. In addition to abdominal wall hernia repair, mesh may be used for repair of diaphragmatic hernias, urinary incontinence in women (female slings), ge… Show more

“…Polyprolene mesh has been used for hernia repair for over 50 years and is considered the standard of care to reinforce the abdominal wall muscles after a hernia. Nonetheless, there are a host of issues with polyprolene mesh, resulting in complications reviewed by Gavlin et al (2020) . Two common complications of synthetic meshes are infection and fibrosis, triggering chronic inflammation and discomfort to the patient ( Wang See et al, 2020 ).…”

“…So far, computed tomography was mainly used to image complications of surgery or mesh repair (25). Here, we present evidence that the tissue elasticity, as a major influence on the durability of repair on the bench test, can be analyzed with the aid of an added Valsalva's maneuver in patients (4).…”

“…Abdominal wall hernias are repaired at open or laparoscopic surgery. Meshes are used to provide a tension-free closure with a lower recurrence incidence [36]. Laparoscopic repair is not recommended in > 10 cm hernias [10,37].…”
