After lymphography from the foot, with uncertain findings, fine-needle biopsy during fluoroscopy was performed in 107 patients. In 69 patients with possible metastases, involvement was demonstrated in 46, confirmed in 43 at operation. Lymph node involvement by general lymphatic disease was suggested in 38 patients, with biopsy confirmation in 12. In metastatic disease non-representative material was obtained in only 2 patients, in general lymphatic disease in 10. No significant complications occurred, and the side effects were small.

Direct unencumbered visualization of areas of interest by image intensification combined with more invasive techniques of percutaneous puncture and angiography have extended the scope of the radiologist in the diagnosis and management of patients with neoplastic diseases. Percutaneous biopsies can be obtained more safely and with greater reliability from lesions of the lungs, skeleton, kidneys, liver, pancreas, and retroperitoneal lymph nodes. Intravascular therapy by arterial occlusion of the neoplasm has been employed to control hemorrhage, to decrease tumor mass, and preoperatively to facilitate surgical removal. The possibility of initiating an immune response to the ischemic neoplasm is also discussed.

In determining the extent of disease in patients with carcinoma, lymphangiography when read as positive has an accuracy of 90 to 95%. When considered negative 15 to 20% prove to have metastatic disease. The recent use of percutaneous transperitoneal aspiration biopsy of previously opacified lymph nodes has enhanced the value of lymphangiography. Eighty percent of the aspiration biopsies yield sufficient tissue for cytologic diagnosis.
