Fibromyalgia syndrome (FMS) is a chronic, idiopathic condition of widespread musculoskeletal pain affecting more women than men. Even though clinical studies have provided evidence of altered central pain pathways, the lack of definitive pathogenesis or reliable objective markers has hampered development of effective treatments. Here we report that the neuropeptides corticotropin-releasing hormone (CRH), substance P (SP), and SP-structurally-related hemokinin-1 (HK-1) were significantly (P 5 0.026, P , 0.0001, and P 5 0.002, respectively) elevated (0.82 6 0.57 ng/ml, 0.39 6 0.18 ng/ml, and 7.98 6 3.12 ng/ml, respectively) in the serum of patients with FMS compared with healthy controls (0.49 6 0.26 ng/ml, 0.12 6 0.1 ng/ml, and 5.71 6 1.08 ng/ml, respectively). Moreover, SP and HK-1 levels were positively correlated (Pearson r 5 0.45, P 5 0.002) in FMS. The serum concentrations of the inflammatory cytokines interleukin (IL)-6 and tumor necrosis factor (TNF) were also significantly (P 5 0.029 and P 5 0.006, respectively) higher (2.97 6 2.35 pg/ml and 0.92 6 0.31 pg/ml, respectively) in the FMS group compared with healthy subjects (1.79 6 0.62 pg/ml and 0.69 6 0.16 pg/ml, respectively). In contrast, serum IL-31 and IL-33 levels were significantly lower (P 5 0.0001 and P 5 0.044, respectively) in the FMS patients (849.5 6 1005 pg/ml and 923.2 6 1284 pg/ml, respectively) in comparison with healthy controls (1281 6 806.4 pg/ml and 3149 6 4073 pg/ml, respectively). FMS serum levels of neurotensin were not different from controls. We had previously shown that CRH and SP stimulate IL-6 and TNF release from mast cells (MCs). Our current results indicate that neuropeptides could stimulate MCs to secrete inflammatory cytokines that contribute importantly to the symptoms of FMS. Treatment directed at preventing the secretion or antagonizing these elevated neuroimmune markers, both centrally and peripherally, may prove to be useful in the management of FMS.

Fibromyalgia (FM) is a syndrome characterized by widespread chronic pain, tenderness, stiffness, fatigue, and sleep and mood disturbances. Current evidences suggest that inflammatory mediators may have an important role in the pathogenesis of fibromyalgia. Every day new evidences emerging of the role of the immune system and the inflammatory process in the pathophysiology of this disease. Thus, the aim of this work has been review that altered inductors, inflammatory mediators and effectors have been reported in fibromyalgia, and its relationship to disease's symptoms. If in fibromyalgia underlies widespread disruption of most characteristic components on the inflammatorý s process (endogenous inductors, mediators and effectors) is logical to think that fibromyalgia is producing a sustained inflammatory response in time, unregulated, responsible for the disease´s symptoms. The prevalence of FM in the world ranges between 0.7% and 3.2%. It is more prevalent on women than men with a ratio women/men 20:1 (4.2% prevalence on women compared with 0.2% on men) [1] and it can develop at any age [2].The exact cause of fibromyalgia is unknown, but it has been proposed that the FM is a stress-related disorder that may involve a dysfunction of the hypothalamic-pituitary-adrenal axis (HPA) [3,4]. Thus, adverse life events, such as emotional, physical and sexual abuse on children and adults have been suggested to contribute to the development of FM [5][6][7].Risk factors FM have been the subject of much debate in recent research. There is no doubt that the experience of an abusive relationship, maintained over time, is a stressful situation that could be related to the presence of FM [8,9].Like all physiological systems, the immune system is under the control of the neuro-endocrine system and interacts and works in concert with other regulatory body systems, thus immune cells respond to neuroendocrine signals through specific receptors for hormones, neurotransmitters and neuropeptides. Therefore, dysregulation of the hypothalamic-pituitary-adrenal axis by chronic stress situations necessarily lead to a dysregulation of the immune function. Thus, inflammation is a characteristic process of the immune system, considered a defense response that is induced by infection or trauma. Resumo: Este trabalho teve como objetivo avaliar o efeito do treinamento aquático sobre as respostas eletromiográficas do músculo trapézio fibras descendentes em indivíduos com fibromialgia. O delineamento do estudo foi clínico experimental, realizado na clínica de fisioterapia da Universidade Federal de Alfenas -MG (UNIFAL-MG). A amostra foi composta por 11 indivíduos com diagnóstico de fibromialgia. Foram realizados quatro momentos de avaliação: inicial, 10, 20 e 30 sessões. O instrumento de avaliação utilizado foi a eletromiografia de superfície para o músculo trapézio descendente bilateralmente em repouso e em contração isometria voluntária máxima (CIVM). As intervenções foram compostas por treinamento aeróbico aquático (aquecimento, condicionamento e resfriamento), 3x por semana, durante dois meses e meio. Os resultados mostratam redução significativa (intragrupo) em ambos os lados para o músculo trapézio fibras descendentes (p=0,001 e p=0,004) e redução significativa (intergrupos) na quarta avaliação, durante o repouso. Conclui-se que o treinamento aquático promoveu alteração na atividade eletromiografia do músculo trapézio fibras descendentes em indivíduos com fibromialgia.
