Identifying Barriers to Delivering the Awakening and Breathing Coordination, Delirium, and Early Exercise/Mobility Bundle to Minimize Adverse Outcomes for Mechanically Ventilated Patients

Abstract: We provide the first, to our knowledge, systematic differential diagnosis of barriers to ABCDE delivery, moving beyond the conventional focus on patient-level factors. Our analysis offers a differential diagnosis checklist for clinicians planning ABCDE implementation to improve patient care and outcomes.

“…Although it may be difficult to observe similarities in barriers when comparing individual studies, the use of a conceptual framework such as CFIR allows implementation researchers to classify barriers into major domains and themes. As Table 1 [10 ▪▪ ,38 ▪▪ ,39 ▪▪ ,4042 ▪ ,48 ▪▪ ,49 ▪▪ ,53 ▪▪ ]demonstrates, the recent literature has identified a multitude of barriers that fall within all five CFIR domains, although most studies have investigated barriers in the inner setting and individuals domains. Having a systematically derived catalog of barriers allows implementation researchers and clinicians to design and evaluate implementation strategies that are targeted correctly, allowing for the greatest chance of success in improving the adoption of evidence-based practice.…”

“…Several recent studies have examined the facilitators and barriers to ABCDE implementation or implementation of individual ABCDE components. Costa et al [38 ▪▪ ] conducted a systematic review to identify barriers to ABCDE implementation (both the full bundle and individual components) and classify them according to CFIR domains. They identified 107 barriers from 49 studies.…”

“…Some are related to the patient, including safety concerns in terms of hemodynamical instability or lack of cooperation. 169 The majority are related to the clinician implementing the bundle, in particular lack of knowledge about its benefits, reluctance to follow guidelines, preference for autonomy, perceived increased workload, 170 and lack of confidence in the bundle's efficacy. [169][170][171] Finally, there are barriers related to the structure of the ICU, including culture and organization, teamwork, physical environment, lack of resources, and inadequate management/leadership.…”

“…Optimizing critical care in intensive care units (ICU) based on evidence‐based practice (EBP), international consensus, and guidelines outlined from all available scientific knowledge is important to enhance patient outcomes . The implementation of EBP remains a challenge and lack of effective communication is one among many other implementation barriers in the ICU . Gathering multi‐professional teams for teaching and training purposes is particularly demanding due to the busy ICU environment and the context in which critical care is provided .…”

“…The implementation of EBP remains a challenge and lack of effective communication is one among many other implementation barriers in the ICU . Gathering multi‐professional teams for teaching and training purposes is particularly demanding due to the busy ICU environment and the context in which critical care is provided . Moreover, traditional electronical professional hospital communication platforms generally require a login procedure onto a hospital server, limiting the accessibility and thus effective real time communication about EBP.…”
