Indonesia is one of countries with staggering number of elderly population. This population is at risk having comorbidities, polypharmacy, inappropriate medication, and adverse drug reactions. Aims: This study aimed to determine the prevalence of Potentially Inappropriate Medications (PIM) and Potentially Prescription Omissions (PPO) among ambulatory geriatric patients. Settings and Design: The research was conducted in an internal medicine clinic of a hospital in Jember Regency, East Java, Indonesia using a retrospective, crosssectional design. Methods and Material: Data were collected consecutively with a targeted sample size of 96 patients during September 2016. Each patient data was traced back for a minimum of three mo period. Analysis of drugs with PIM and PPO was based on criteria of the Screening Tool of Older Person's Prescription (STOPP) and Screening Tool to Alert to Right Treatment (START) version 2. Statistical analysis used: Descriptive statistics were used to report the results. Results: The results showed that a total of 92 PIM events occured in 64 patients (64 %) and were found more in females (66 %), aged 65 yr to 69 yr (70 %) with glimepiride and pioglitazone as the first and second leading drugs causing PIM events. All eight PPOs were in the form of not giving antihypertensive therapy to hypertensive patients according to the START criteria. Conclusions: In conclusion, the PIM figures were large, while the PPO was small and narrowed to one problem. Increasing alertness and caution in administering drug therapy will be very necessary to reduce adverse drug reactions in geriatric patients.

Geriatri cendrung memiliki lebih dari satu penyakit yang menyebabkan pemberian resep polifarmasi. Polifarmasi merupakan faktor terjadinya peresepan obat tidak tepat. Kriteria STOPP START dapat digunakan untuk mengidentifikasi peresepan obat yang tidak tepat meliputi Potentially Inappropriate Medications (PIM) dan Potentially Prescribing Omissions (PPO) pada pasien geriatri. Tujuan dari penelitian ini adalah mengetahui karakteristik pasien dan prevalensi potensi terjadinya PIM dan PPO dalam peresepan obat berdasarkan kriteria STOPP START pada pasien geriatri yang dirawat inap dengan penyakit Kardiovaskular di salah satu rumah sakit Balikpapan periode Januari-Desember 2021. Penelitian ini merupakan penelitian observasional dengan data retrospektif dengan teknik pengambilan data secara purposive sampling. Hasil penelitian yang diperoleh dari 55 data pasien dengan karakteristrik berdasarkan pasien geriatri paling banyak pada 60-74 tahun (65,45%), berdasarkan jenis kelamin paling banyak adalah laki-laki (58,18%), berdasarkan penggunaan jumlah obat paling banyak adalah 5-10 obat (65,45%), dan penyakit kardiovaskular paling banyak adalah angina tidak stabil (34,55%). Prevalensi potensi terjadinya PIM sebesar (32,72%) dan PPO sebesar (3,64%) pada pasien geriatri rawat inap dengan penyakit kardiovaskular di salah satu rumah sakit Balikpapan.

Adverse Drug Events (ADE) are closely related to Potentially Inappropriate Medications (PIMs) among the elderly, and can cause an increase in morbidity, mortality, and medical cost. The tools often used to assess PIMs include Beers and Screening Tool of Older Persons’ Prescriptions (STOPP) Criteria. Therefore, this study aimed to compare the PIMs identification using Beers 2019 and STOPP version 2 2016 Criteria. A descriptive-analytical method was used, and data were collected retrospectively from the medical records of 340 patients at Pondok Kopi Islamic Hospital period in 2018. The results showed that among 324 patients who met the inclusion criteria with Beers 2019, PIMs were present in 136 (41.85%), with 181 cases. Most of them were on criteria 3, namely 46 (25.41%) and 33 (18.23%) cases of furosemide and spironolactone, respectively. Meanwhile, out of 308 patients who met the inclusion criteria with STOPP, PIMs were found in 14 (4.55%) with a total of 18 cases. The use of aspirin or clopidogrel was observed in 7 people (38.89%) with uncontrolled severe hypertension, while NSAID usage was found in 3 uncontrolled severe hypertension patients (16.67%). Furthermore, there were 3 cases of aldosterone antagonist or ARB usage, which was combined with other drugs that can increase potassium without proper monitoring of the serum level (16.67%). In Pondok Kopi Islamic Hospital, Beers 2019 criteria can describe PIMs data better than the STOPP version 2 2016 due to data availability. Clinician and pharmacist collaboration is also needed in formulating the critical supporting data.
