It is not clear whether muscle oxygenation (O(2-NIRS)) measured by near-infrared spectroscopy (NIRS) correlates with femoral venous SO2 (S(fv)o2) during normoxic exercise. Therefore, the purpose of this study was to compare physiologically calibrated O(2-NIRS) with S(fv)o2 in subjects performing one-legged dynamic knee extension exercise (1L-KEE). Five healthy male subjects (age 25+/-2 year, height 177.8+/-4.8 cm, body weight 67.1 +/- 5.0 kg; mean +/- SD) performed 1L-KEE at 20, 40, and 60% of peak work rate (WR-peak) each for 4 min. S(fv)o2 was measured at rest and during the 3rd minute of each work rate. O(2-NIRS) was continuously monitored in a proximal region of the vastus lateralis (VL-p), a distal region of VL (VL-d), and a proximal region of the rectus femoris (RF-p). S(fv)o2 was 56.0% at rest and decreased to 36.6 at 20% WR-peak, 35.8 at 40% WR-peak, and 31.1 at 60% WR-peak. There was a significant correlation between O(2-NIRS) and S(fv)o2(VL-p: r (2) = 0.62, VL-d: r2 = 0.35, RF-p: r2 = 0.62, with a moderate variation among individuals at each site; residual values = 4.83 - 11.75). These data indicate that NIRS measurement provides a reflection of S(fv)o2 during 20-60% WR-peak of normoxic 1L-KEE.

During exercise, the metabolic need for oxygen in muscles increases many times above the resting level. To meet this oxygen demand, oxygen supply to the contracting muscle must increase; this is accomplished by an increased cardiac output and a redistribution of the blood supply. Six healthy subjects performed the standing plantar flexion exercises for 2 min; the frequency was one contraction per second. The NIR imaging device was placed over the medial head of the right gastrocnemius muscle and the signals from two optical sensors situated on the middle proximal and middle distal portions were used. The NIR-O 2 saturation (difference between deoxygenated and oxygenated Hb signals) and NIR-blood volume (sum of the oxygenated and deoxygenated Hb signals) were calculated in optical density units. Plantar flexion resulted in more deoxygenation during exercise and more reoxygenation during recovery in the distal portion compared with the proximal portion. The changes in NIR-O 2 between rest and a 2 min exercise, and between a 2 min exercise and a 3 min recovery were 0.11 and Ϫ0.23, respectively, in the distal portion, which were significantly larger than proximal values (0.05 and Ϫ0.10, pϽ0.05). Plantar flexion resulted in lower NIR-blood volumes during exercise and greater recovery of blood after exercise in the distal portion compared with the proximal portion. The changes in NIR blood volume between rest and a 2 min exercise and between a 2 min exercise and a 3 min recovery were Ϫ0.19 and 0.31, respectively, in the distal portion, significantly larger than proximal values (Ϫ0.07 and 0.12, pϽ0.05 for all comparisons). These findings indicate that the distal portion of the medial gastrocnemius had larger changes in NIR-O 2 saturation and NIR-blood volume than the proximal portion had. This is consistent with the distal portion having a greater impairment of blood flow possibly because of the higher intramuscular pressure during exercise. In conclusion: (1) regional differences in oxygen status in the gastrocnemius muscle were detected with exercise, with the distal portion having greater NIR-O 2 saturation and NIR-blood volume changes, and (2) the NIR imaging device might be a useful method to detect the regional differences of oxygen status in the muscle.

Being the closest layer to our body, textiles provide an ideal platform for integrating sensors and actuators to monitor physiological signals. We used a woven textile to integrate photodiodes and light emitting diodes. LEDs and photodiodes enable near-infrared spectroscopy (NIRS) systems to monitor arterial oxygen saturation and oxygenated and deoxygenated hemoglobin in human tissue. Photodiodes and LEDs are mounted on flexible plastic strips with widths of 4 mm and 2 mm, respectively. The strips are woven during the textile fabrication process in weft direction and interconnected with copper wires with a diameter of 71 m in warp direction. The sensor textile is applied to measure the pulse waves in the fingertip and the changes in oxygenated and deoxygenated hemoglobin during a venous occlusion at the calf. The system has a signal-to-noise ratio of more than 70 dB and a system drift of 0.37% ± 0.48%. The presented work demonstrates the feasibility of integrating photodiodes and LEDs into woven textiles, a step towards wearable health monitoring devices.
