Background: Previous studies showed that thyroid dysfunction in women with gestational hypertension could negatively affect maternal and fetal outcomes. In this study, we aimed to investigate whether thyroid dysfunction assessed in the second half trimester contributed to neonatal outcomes of pregnancy in different subtypes of gestational hypertension disease. Methods: We performed a retrospective case-control study and collected data from 135 singleton pregnant women with gestational hypertension disease and their offspring who delivered in Renmin Hospital of Wuhan University from January 2015 to June 2017. We classified the patients based on the severity of the preeclampsia into three groups: pregnant induced hypertension (PIH), mild preeclampsia (MPE) and severe preeclampsia (SPE). Based on the onset time of preeclampsia, we classified the patients into PIH, early onset preeclampsia (EPE) and late onset preeclampsia. Demographic data and levels of thyroid hormones, as well as the adverse maternal and neonatal outcomes were collected from Electronic Medical Records. Logistic regression was used to estimate the associations between thyroid dysfunction and neonatal outcomes in these patients. Results: Gestational weeks and neonatal birthweight were significantly lower, while incidence of preterm birth was significantly higher in the SPE and EPE groups than those in the PIH group (P < 0.001). Thyroid dysfunction was more frequent in the SPE group than in the MPE group (P = 0.01). Incidences of both preterm birth and low birth weight were significantly higher in patients with thyroid dysfunction (P = 0.008, P = 0.047 respectively). After adjustment, both severity of gestational hypertension (OR = 4.360, 95%CI [2.050, 9.271], P < 0.001; OR = 4.023, 95%CI [1.933, 8.372], P < 0.001) and thyroid dysfunction (OR = 3.011, 95%CI [1.248, 7.262], P = 0.014; OR = 11.306, 95%CI [1.040, 122.889], P = 0.046) were associated with higher risk of preterm birth and low birth weight, while the onset time of preeclampsia (OR = 0.031, 95%CI [0.009, 0.110], P < 0.001; OR = 0.097, 95%CI [0.033, 0.282], P < 0.001) was negatively associated with the risk of preterm birth and low birth weight. Conclusion: Severe and early onset preeclampsia, as well as thyroid dysfunction are associated with higher risk of preterm birth and low neonatal birth weight. Therefore, our data suggest that monitoring thyroid hormones in women with preeclampsia might help to predict adverse neonatal outcomes.

Objective: Preeclampsia is a potentially fatal condition during pregnancy and following delivery. Numerous organs' functions can be influenced by preeclampsia. It is crucial, as this illness may be linked to abnormalities in thyroid function. Therefore, the aim of this study was to assess the frequency of thyroid dysfunction and its association with hypertension in pregnant women. Methodology: This descriptive cross sectional study was conducted at Obstetrics and Gynecology Unit-I, Civil Hospital, Karachi, using a non-probability consecutive sampling technique. The duration of the study was about six months from October 2022 till March 2023. A total of 80 pregnant women who were between the ages of 18 and 40, had gestational ages between 20 and 40 weeks, and had laboratory results that suggested hypothyroidism were included in this study. A chi-square test was applied to evaluate the association between maternal age, parity, gestational age, serum levels of TSH and T4 and pregnancy induced hypertension. Results: The study findings showed that most of the women 33(41.25%) were between 26 to 35 years of age. The mean age of the pregnant women was 29.43±4.64 years. The mean parity was 1.74±0.83 and the mean gestational age was 30.33±5.22 weeks, and the mean duration of hypothyroidism was 4.27±2.06 years. Concerning thyroid parameters, the mean T4 was 5.79±1.21 pmol/L, and the mean TSH was 6.28±2.03 mIU/L. Additionally, there was an insignificant association observed between pregnancy induced hypertension and maternal age, gestational age, parity, and thyroid hormone parameters. Conclusion: This study concluded that alterations in the thyroid function of pregnant women led to pregnancy induced hypertension. Only 25% of hypothyroid pregnant women had gestational hypertension. Furthermore, age, parity, gestational age, and thyroid parameters of pregnant women were insignificantly associated with pregnancy induced hypertension. Gestational hypothyroidism may lead to preeclampsia development. However, this pathophysiological is unknown. We expect to find a shared mechanism by comparing hypothyroidism and preeclampsia. From our transcriptome data, we recognized olfactory receptors as that fingerprint. The reduction of taste and smell in hypothyroid patients has been known for a long time. Therefore, we decided to look to the olfactory receptors and aimed to identify genes capable of predicting preeclampsia (PEC). Methods: An Ion Proton Sequencer (Thermo Fisher Scientific, Waltham, MA, USA) was used to construct the transcriptome databases. RStudio with packages Limma v.3.50.0, GEOquery v.2.62.2, and umap v.0.2.8.8 were used to analyze the differentially expressed genes in GSE149440 from the Gene Expression Omnibus (GEO). The 7500 Real-Time PCR System (Applied Biosystems, Foster City, CA, USA) was used for RT-qPCR amplification of OR6X1 and OR4E2. Results: Our transcriptomic datasets analysis revealed 25.08% and 26.75% downregulated olfactory receptor (ORs) in mild nontreated gestational hypothyroidism (GHT) and PEC, respectively. In the GSE149440 GEO dataset, we found OR5H1, OR5T3, OR51A7, OR51B6, OR10J5, OR6C6, and OR2AG2 as predictors of early-onset PEC. We also evaluate two chosen biomarkers’ responses to levothyroxine. The RT-qPCR demonstrated a difference in OR6X1 and OR4E2 expression between GHT and healthy pregnancy (p < 0.05). Those genes presented a negative correlation with TSH (r: −0.51, p < 0.05; and r: −0.44, p < 0.05), a strong positive correlation with each other (r: 0.89; p < 0.01) and the levothyroxine-treated group had no difference from the healthy one. We conclude that ORs could be used as biomarkers at the beginning of gestation, and the downregulated ORs found in GHT may be improved with levothyroxine treatment.
