Hypophyso-Gonadal Function in the Cryptorchid Child: Differences Between Unilateral and Bilateral Cryptorchids

Abstract: In 22 normal boys, 33 unilateral and 14 bilateral cryptorchids, a gonadal function test (2000 IU of HCG im each a day for three days and assays of plasma testosterone and plasma oestradiol-17\g=b\ before and after the HCG administration) as well as an LH-RH test were carried out. In 60% of the cases, both normal and cryptorchid boys, plasma oestradiol-17\g=b\(both in basal conditions and after stimulus) were found to be less than the sensitivity (5 pg/ml) of the method. While the plasma testosterone was simila… Show more

“…Cacciari et al (1976) have not found any significant abnormalities of response to LH-RH in unilateral cryptorchids. Laron et al (1975) have shown that unilateral operated cryptorchids with compensatory contralateral testicular hypertrophy have basal plasma FSH levels and LH and FSH response to LH-RH greater than controls.…”

“…Illig et al [10] with a treatment schedule similar to Happ's method, carried out for four weeks, obtained complete descent of testes in 38% of cases. Conventional therapy with hCG has been described as reaching positive results varying from 200/0-90% [4].…”

“…Patients with Klinefelter's syndrome, when diagnosed before the onset of puberty, have usually normal re sponses to LH-RH [9], In cryptorchid prepubertal boys, a significant decrease of LH response to LH-RH has been found, while their FSH response was normal [10]. How ever, these data do not agree completely with those found in other clinics [11,12], The administration of HCG induced about a 7-fold increase in testosterone in normal prepubertal boys [13]. Before the age of 12 years, the peak plasma testosterone response to HCG stimulation was within the normal range in Klinefelter's syndrome [9].…”
