Hypertrophic pulmonary osteoarthropathy in Hodgkin's disease. Reversal with chemotherapy

Abstract: A case of nodular sclerosis Hodgkin's disease, stage IVB, with lung involvement and hypertrophic pulmonary osteoarthropathy (HPOA), was treated with quadruple cytotoxic chemotherapy. The pulmonary lesions, which were thin walled cavities, and the lymphadenopathy resolved completely after two courses of chemotherapy. The clubbing and all evidence of periosteal new bone formation disappeared after six courses of treatment. Complete reversal of the syndrome by chemotherapy has not been previously described. “…Following successful treatment of the primary disorder, finger clubbing has been known to reverse. [2][3][4] Finger clubbing has not been previously reported in association with acute myeloid leukaemia but two cases associated with chronic lymphocytic leukaemia have been documented.35 A case of acute myeloid leukaemia with marked finger and toe clubbing, and a likely intrathoracic granulocytic sarcoma at presentation is described.…”

“…Five of the nine patients were from Turkey, which in itself is remarkable as the peak incidence of HL in Turkey is before 10 years of age, and two thirds of the patients have mixed cellularity type of disease [5]. A review of 14 adult patients with HL and HOA revealed that only three had the nodular sclerosing type of the disease [3]. However, current data are insufficient to reveal an agedependent distribution of HL types in patients with associated HOA.…”
