A recent editorial by Shah (1975) outlined the eventful and controversy-filled saga of hypertrophic obstructive cardiomyopathy (HOCM) (idiopathic hypertrophic subaortic stenosis, muscular subaortic stenosis, obstructive asymmetric septal hypertrophy). Concepts of the anatomical and pathophysiological mechanism(s) of left ventricular outflow obstruction present in this disease have evolved slowly over the years. At first, obstruction was attributed to a circular muscle band in the left ventricular outflow tract (Brachfeld and Gorlin, 1959;Brock, 1959;Cohen et al., 1964) but later this view was felt to be too simple, overlooking the fact that the left ventricle unlike the right does not have a true muscular outflow tract (Shah, 1975). Whether a true intraventricular pressure gradient was actually even present in hypertrophic cardiomyopathy was subsequently questioned by Criley et al. (1965). They explained recorded subaortic pressure gradients as being caused by entrapment of the catheter, based on the demonstration that high subendocardial pressures transmitted to a catheter embedded in the trabeculated left ventricular apex result in artefactual gradients (Morrow et al., 1965;Dieudonne, 1966 More recently, echocardiographic observations have provided additional information regarding the possible causes of systolic outflow obstruction in hypertrophic obstructive cardiomyopathy. The septum is disproportionately thicker than the posterior wall by a ratio of 1-3 or more (Henry et al., 1973a) and the anterior mitral leaflet moves in systole toward the septum (Shah et al., 1969). The length and degree of the systolic apposition of the mitral leaflet against the septum have correlated well with outflow gradients found at catheterisation (Henry et al., 1973b), though lack of correlation has been noted by other authors (Rossen et al., 1974). The precise mechanisms for the systolic apposition of the anterior mitral leaflet against the septum are not clearly understood, being attributed to malalignment of the anterior papillary muscle (Reis et al., 1974) or to a Venturi effect on the left ventricular outflow tract (Wigle et al., 1971) produced by the abnormal anatomy and contraction patterns. A miocardiopatia hipertrófica (MH) caracteriza-se por apresentar hipertrofia simétrica ou assimétrica do miocárdio ventricular, geralmente com predomínio no septo interventricular (SIV), com ou sem obstrução dinâmica de via de saída [1][2][3] . A cavidade ventricular costuma ser de tamanho normal ou reduzida e apresenta disfunção diastólica resultante, principalmente, de déficit do relaxamento ventricular, mas também devido ao aumento da rigidez intrínseca da parede e da redução da câmara ventricular 4,5 . A função contrátil costuma ser normal ou hiperdinâmica. A MH pode se apresentar desde uma forma assintomática até situações de insuficiência cardí-aca avançada ou de morte súbita, muitas vezes, sem nenhuma sintomatologia prévia, tornando o seu diagnóstico precoce difícil, porém, imprescindível em alguns casos 6 . A ecocardiografia é o exame mais empregado atualmente na avaliação da MH. Tem sido empregada tanto na investigação de casos suspeitos quanto para screening de parentes de pacientes com a doença. Através da ecocardiografia é possível avaliar os aspectos morfológicos, funcionais e hemodinâmicos da MH 7 . A participação do ecocardiograma no estudo das miocardiopatias (MC) é cada vez mais decisiva em sua caracterização e no esclarecimento dos mecanismos fisiopatológicos envolvidos, com importantes implicações terapêuticas e prognósticas sendo, atualmente, o método de maior aplicabilidade nas várias formas de MC. Apesar do ecocardiograma ter alta sensibilidade para detectar a MC, tal constatação não é, em muitos casos, específica para este diagnóstico. Na prática, o diagnóstico de MC é realizado após a exclusão de todas as demais doenças cardiovasculares, que podem provocar dano miocárdico secundário, contudo, deve ser lembrado que essas doenças podem coexistir com uma MC. Aspectos ecocardiográficosCritérios diagnósticos -O achado ecocardiográfico característico na MH é a hipertrofia do ventrículo esquerdo (VE), porém, há uma grande variabilidade no seu grau e padrão. A maioria dos pacientes (90%) apresenta a forma assimétrica clássica com predomínio no SIV 8 (tab. I). As demais paredes do VE costumam apresentar graus variáveis de hipertrofia, exceto a parede posterior (PP) que costuma ter espessura normal 9 . Nesta forma, o SIV geralmente apresenta uma espessura >15mm (normal <10-11mm) e a relação SIV/PP é >1,3, fato pelo qual a MH é, algumas vezes, indevidamente chamada de hipertrofia septal assimétrica (HSA) 10,11 . Mesmo porções contíguas do VE podem apresentar diferentes espessuras e a transição entre elas é, freqüen-temente, abrupta. Dessa forma, em adultos, o diagnóstico de MH pode ser feito a partir da espessura da parede do VE, quando esta for >13mm 12 . Espessura de parede entre 11 e 13mm com ou sem relação SIV/PP >1,3 e sem causa aparente pode corresponder à forma incipiente de MH, particularmente, quando há disfunção diastólica associada 13,14 .
