Background COVID-19 patients usually present multiple comorbidities and complications associated with severe forms of SARS-CoV-2 infection. This study aimed to assess the risk factors and prevalence of comorbidities and complications contributing to the severity of COVID-19. Methods This meta-analysis was performed according to PRISMA guidelines. We searched various databases, including PubMed, Google Scholar, and Scopus (between 2020 and 2023), for eligible studies for this meta-analysis. Results Thirty-three studies were eligible, including 85,812 patients, of which 36 % (30,634/85,812) had severe disease, whereas 64 % (55,178/85,812) had non-severe disease. Severe cases were potentially correlated with the following factors: gender (male) (odd ratio (OR) = 1.52, 95 % CI: 1.34–1.73), advanced age (OR = 3.06, 95 % CI: 2.18–4.40) pre-existing smoking (OR = 1.33, 95 % CI: 1.01–1.75), obesity (OR = 2.11, 95 % CI: 1.47–3.04), diabetes (OR = 1.81, 95 % CI: 1.35–2.43), hypertension (OR = 2.22, 95 % CI: 1.72–2.87), coronary heart disease (OR = 2.17, 95 % CI: 1.42–3.31), CKD (OR = 2.27, 95 % CI: 1.26–4.06), COPD (OR = 1.95, 95 % CI: 1.22–3.09), malignancy (OR = 1.63, 95 % CI: 1.07–2.49) and cerebrovascular disease (OR = 2.76, 95 % CI: 1.63–4.62). All these comorbidities were significantly higher in the severe COVID-19 group compared with the non-severe COVID-19 group. In addition, the most severe complications were associated with shock (OR = 28.08, 95 % CI: 3.49–226.03), ARDS (OR = 13.09, 95 % CI: 5.87–29.18), AKI (OR = 16.91, 95 % CI: 1.87–152.45) and arrhythmia (OR = 7.47, 95 % CI: 2.96–18.83). However, these complications were the most likely to prevent recovery in patients with severe affections compared with non-severe affection groups. Conclusion All the comorbidities and complications listed above are more likely to cause severe forms of COVID-19 in some patients and hinder recovery. They are therefore risk factors to be controlled to minimize the undesirable effects of the disease.

The COVID-19 infection has highlighted the most vulnerable patients. Indeed, COVID-19 patients suffering from another pathology including NCDs such as Arterial Hypertension (Hypertension), diabetes, cancers and respiratory diseases are paying a heavy price for this pandemic. We undertook a study in Niger to better document this comorbidity in a cross-sectional study that brought together patients hospitalized at the Niamey General Reference Hospital for COVID-19 infection and suffering from one or more NCDs. Among 273 patients hospitalized from March 19 to June 03, 2020, 34.8% had a non-communicable disease associated with COVID-19. The average age of the patients was 55 years (22 years to 94 years) and the sex ratio was 2.64 (72.5 % men and 27.5% women). Hypertension was the most represented NCD with 24.5%, followed by diabetes in 17.9% of cases, respiratory diseases 3.66% and other diseases (Heart disease, Obesity, Dyslipidemia, Gout, Chronic renal failure) with 3%. Health workers were the most affected by the disease with 38.6% of cases. The average consultation time was 3.77 days with extremes ranging from 1 to 8 days. The clinical symptoms characterizing the two main groups of patients (hypertensive and diabetic) were almost identical. It consisted mainly of cough, fever, chills, sore throat and rhinorrhea. According to the WHO clinical criteria for the severity of COVID-19, 34 patients or 16.11% were severe and 177 patients or 83% moderate. The clinical severity of the disease is significantly correlated with the patient's age (over 50 years) and the presence of an NCD associated with COVID-19. A total of 35.8% were hospitalized in intensive care in the NCD and COVID-19 group and 14.6% in the NCD group (p <0.001). The average length of patient hospitalization was 6.87 days overall, it was 7 days in intensive care. In 61.7% of cases the length of hospitalization was greater than 4 days. There was 22.1% in the NCD group and 7.3% in the just COVID-19 group (p <0.001). A total of 90.6% of registered deaths occurred in intensive care.

Introduction: Flemingia faginea, a Fabaceae family medicinal plant, has been used for a long time in Burkina Faso for the treatment of hypertension and excess salt. However, the safety of the preparations derived from this plant has not yet been scientifically documented. This study aimed to evaluate the acute and subacute oral toxicity of the leafy stems aqueous decoction of F. faginea (FAD) in healthy normotensive mice and rats and the impact on their normal blood pressure. Material and Methods: The acute oral toxicity study was conducted according to the toxicity class method of the Economic Cooperation and Development Organization (OECD) guideline 423. Subacute toxicity was carried out according to the OECD Guideline 407 for repeated dose chemical toxicity for 28 days. Hematological and biochemical analyzes of blood were performed after autopsy. An evaluation of the impact of the extract on the blood pressure of rats was performed using the non-invasive method. Results: A single oral dose of 2000 mg/kg bw to mice did not cause mortality or clinical signs or symptoms of toxicity during the 14-day study. The FAD was classified in the fifth category of the Harmonized System of Classification of the United Nations and considered practically safe with an estimated 50% lethal dose of 5000 mg/kg bw. Daily gavage of male and female rats with doses of 100,500 and 1000 mg/kg did not result in mortality or significant adverse effects during the 28days of experimentation. There were no significant differences in body weight gain, food &water consumption or relative vital organ weights in treated animals. Analysis of the hematological and biochemical parameters of blood serum did not show significant differences between treated and control animals in this study. Additionally, no aberrant changes were found in the systolic and diastolic blood pressures of the test animals during the 28 days of inclusion compared to those of the control group. Conclusion: The extract FAD could be considered safe within the doses tested for the results of the toxicological evaluation. However, microscopic, histopathological, and subchronic investigations will have to be carried out to confirm the safety of this extract use.
