Hypersensitivity pneumonitis among wind musicians – an overlooked disease?

Abstract: Hypersensitivity pneumonitis (HP) is a complex pulmonary disorder mediated by the immune system and caused by various inhaled antigens against which the subject has previously been sensitized. In about 50% of the cases, the antigen is not identified. Identification and removal of the eliciting antigen is important for the prognosis. We report two cases of HP caused by molds and atypical mycobacteria isolated from wind instruments. We present the first case of HP caused by bassoon playing and another case of HP … Show more

“…Mycobacterium chelonae was isolated from a bassoon played by a professional musician diagnosed with HP, but molds were also isolated and no NTM or molds were isolated from the BAL from the patient (Moller et al, 2017). Thus, the association may not be causal.…”

“…These observations are corroborated by a case study, published in 2017, where molds and mycobacteria isolated from wind instruments were associated with 2 cases of HP. 35 The first case, was a 56-year-old woman that played the bassoon. Her respiratory symptoms had disappeared during a summer holiday, where she had not been playing.…”

“…In addition to type 1 allergies, mold can also induce type 3 and 4 hypersensitive reactions, known as hypersensitivity pneumonitis (HP) [36]. These reactions are mediated by immune complexes and T helper 1 cells, respectively [37,38].…”
