Hyperintense signal alteration in the suprapatellar fat pad on MRI is associated with degeneration of the patellofemoral joint over 48 months: data from the Osteoarthritis Initiative

Abstract: Hyperintense signal alteration in the suprapatellar fat pad on MRI is associated with degeneration of the patellofemoral joint over 48 months: data from the Osteoarthritis Initiative. Results Baseline SPFP signal alterations were found in 51% of the subjects (n = 217), of whom 11% (n = 23) additionally had a mass effect. Progression of cartilage lesions was significantly higher in subjects with signal alteration versus without (adjusted mean increases, 95% CI; patella: 0.29, −0.07 to 0.64 vs −0.04, −0.40 to 0.… Show more

“…Second, the degree of suprapatellar fat pad (SPFP or quadriceps fat pad alterations) abnormality was assessed on a scale from 0 to 3 as described in figure legend 2 [Fig. 2(b)] 31 . Third, the extent of effusion-synovitis was scored from 0 (no effusion) to 3 as the anterioreposterior effusion diameter visible on sagittal fat saturated CUBE images as previously described 30 and outlined in detail in the legend of Fig.…”

“…Only intra-reader reproducibility was assessed as all the readings had been performed by the same reader. Overall, measures showed substantial to good agreement with intra-reader ICCs of !0.80 for the synovial proliferation subfeature, the IPFP subfeature, the SPFP subfeature 31 , and the effusion-synovitis subfeature 30 . As the reproducibility of T1r and T2 measures had been widely validated with ICCs >0.96 and CVs of <3% 35e38 and as the segmentations for this study were only performed by one segmentator we did not explicitly assess intra-reader reproducibility for T1r and T2 measures in this study.…”

“…The clinical significance of abnormalities in suprapatellar fat pads on MRI is unknown. A recent study based on longitudinal data from OAI showed that in knees with no radiographic OA the presence of suprapatellar fat pad edema was predictive of cartilage loss and BMLs in PFJ over 4 years 32 .…”
