Summary What is known and background: The introduction of vaccines has lead to a significant reduction in morbidity and mortality from diseases such as measles, rubella and poliomyelitis, as well as the eradication of smallpox (Ertl HC, Xiang Z (1996) The Journal of Immunology, 156, 3579–3582). A recent vaccine approved by the Food and Drug Administration (FDA) is the recombinant quadrivalent human papillomavirus (HPV) vaccine (Merck, Gardasil®). Concerns raised with this preventive measure include safety and efficacy issues as well as the financial implications. Furthermore, the use of the vaccine in women outside the currently approved age ranges and in adolescent boys and men has also been a source of debate. Objective: A review of two licensed HPV vaccines (Gardasil, Merck and Cervarix, GalxoSmithKline) in the light of these issues. Methods: Literature searches were conducted using the MEDLINE (1966 – December 2008) and PubMed databases in addition to the Centers for Disease Control and Prevention website. Bibliographies of selected references were also evaluated for relevant articles. Published guidelines and press releases were utilized as were the manufacturer’s package inserts. The collection of information for this review was limited to the most recently available human data. Results and discussion: The HPV quadrivalent vaccine has been effective in the management of HPV by preventing vaccine subtype‐related persistent infection and precancerous lesions as evidenced by numerous clinical trials. It is also regarded as a generally safe and well‐tolerated vaccine, based on an assessment of reported adverse events submitted through governmental databases and analyzed by independent researchers. The majority of adverse events were non‐serious and the vaccine has not been conclusively implicated with serious events. The FDA continues to focus on routine post‐marketing surveillance monitoring of reported adverse events. The bivalent vaccine has also been shown to be effective in reported trials. Its adverse effect profile also appears acceptable. What is new and conclusion: The HPV vaccines appear safe and effective. Additional clinical research on the vaccines on women outside the currently approved age ranges and in males is necessary. Studies on longer‐term outcomes, including cervical cancer and the emergence of new viral genotypes are also necessary.

Both the prophylactic human papillomavirus (HPV) vaccines, Gardasil® and Cervarix®, are licensed for the prevention of cervical cancer in females, and Gardasil is also licensed for the prevention of genital warts and anal cancer in both males and females. This review focuses on the uptake of these vaccines in adolescent males and females in the USA and the barriers associated with vaccine initiation and completion. In the USA in 2009, approximately 44.3% of adolescent females aged 13–17 years had received at least one dose of the HPV vaccine, but only 26.7% had received all three doses. In general, the Northeast and Midwest regions of the USA have the highest rates of HPV vaccine initiation in adolescent females, while the Southeast has the lowest rates of vaccine initiation. Uptake of the first dose of the HPV vaccine in adolescent females did not vary by race/ethnicity; however, completion of all three doses is lower among African Americans (23.1%) and Latinos (23.4%) compared with Caucasians (29.3%). At present, vaccination rates among adolescent females are lower than expected, and thus vaccine models suggest that it is more cost-effective to vaccinate both adolescent males and females. Current guidelines for HPV vaccination in adolescent males is recommended only for “permissive use,” which leaves this population out of routine vaccination for HPV. The uptake of the vaccine is challenged by the high cost, feasibility, and logistics of three-dose deliveries. The biggest impact on acceptability of the vaccine is by adolescents, physicians, parents, and the community. Future efforts need to focus on HPV vaccine education among adolescents and decreasing the barriers associated with poor vaccine uptake and completion in adolescents before their sexual debut, but Papanicolau screening should remain routine among adults and those already infected until a therapeutic vaccine can be developed.

Objetivo: Describir los hallazgos microscópicos e histopatológicos de la citología vaginal y de la colposcopia en mujeres asistentes a la clínica la ASSBASALUDE.S.E y a la LIGA CONTRA EL CÁNCER, seccional Caldas en la ciudad de Manizales (Colombia),y la relación existente entre los factores de riesgo para el cáncer de cérvix e infección por VPH. Materiales y métodos: Estudio de corte transversal basado en analizar aproximadamente1500 historias clínicas de mujeres sexualmente activas, revisadas en ASSBASALUD E.S.E y la LIGA CONTRA EL CÁNCER (Manizales, Colombia, Suramérica), evaluando las características socio-demográficos de las pacientes, sus antecedentesginecobstétricos y los hallazgos histológicos y de colposcopia.Resultados: Edad promedio de inicio de relaciones sexuales de 17.9, Del 19.3% que reportó antecedentes de enfermedades de transmisión sexual, 73.3% fue por VPH, sólo un 4.3% usó anticonceptivo de barrera,42.1% de las citologías se reportaron como normales y 70.5 % de las colposcopias negativas. Hubo 36.5% casos de LEIBG y 25.5% de reportes de VPH por colposcopia. La incidencia de cáncer de cuello uterinofue de 2.3% por colposcopia y se halló 8.8% de LEIAG.
