Background: The objective of the study was to estimate the risk of occupational skin diseases among workers who were exposed to animal manure during rice cultivation in India. Methods: Questionnaire based six month follow up study with weekly assessment of skin diseases in rice field workers from village Dabra, in Haryana state of India who were exposed to animal manure was carried out among the agricultural workers working in rice field along with dermatological examination of exposed parts. Results: Out of total 574, workers, 90 were excluded as per exclusion criteria. 71 workers out of 370 workers who were free from diseases at baseline survey data develop a new disease with cumulative incidence of 19.1%. There was higher prevalence of dermatitis among all of reported skin diseases. Conclusions: Exposure to animal manure mixed with standing water is a major risk factor among the workers working in rice field and attempt should be made to identify the type of dermatitis and relate its causative agent with any specific biological or chemical agents present in animal manure mixed with standing water of rice field.

Objective: The aim of this study was to investigate the effects of the working environment on hand dermatitis (HD) prevalence among nurses in a Chinese teaching hospital.Methods: We utilised a previously validated, self-reporting survey which was translated into Chinese.Results: The prevalence of HD among nurses was 18.3%, although this rate varied between departments, ranging from 9.4% in gynaecology to 26.7% in intensive care (P for Trend=0.3167). Logistic regression indicated that wet work was the most important HD risk factor, with a 9-fold increase (OR 9.0, 95%CI 1.2−74.9, P=0.0342). Allergic disease was also related to HD, with a 4.6-fold increase noted (OR 4.6, 95%CI 1.4−15.0, P=0.0096).Conclusions: Overall, the prevalence of HD among Chinese hospital nurses appears to be less than that of their foreign counterparts. Nevertheless, the burden of this disease does seem to vary with respect to department of employment within the hospital.

Irritant contact dermatitis is often found on the hands of healthcare workers and is generally caused by frequent hand washing, gloves, aggressive disinfectants or detergents. Alcohols have only a marginal irritation potential, although they may cause a burning sensation on pre-irritated skin. A burning sensation when using alcohols therefore, suggests that the skin barrier is already damaged. Two options for hand hygiene are generally available in clinical practice: (1) hand washing with some type of soap and water or (2) hand disinfection with alcohol-based hand rubs. Most clinical situations require the use of an alcohol-based hand rub for decontamination, which is especially useful for reducing the nosocomial transmission of various infectious agents. Washing one's hands should be the exception, to be performed only when they are visibly soiled or contaminated with proteinaceous material, or visibly soiled with blood or other body fluids. The overall compliance rate in hand hygiene is around 50%, which is far too low. In addition, healthcare workers quite often wash their hands with soap and water, when they should use an alcohol-based hand rub. This not only adds to the degree of skin irritation, but is also potentially dangerous for patients, due to the low efficacy of hand washing when compared to hand disinfection with alcohol rubs. Adhering to evidence-based hand hygiene protocols and following international guidelines on hand hygiene practices therefore, can help prevent irritant contact dermatitis among healthcare workers.
