How far should we go in optimal cytoreductive surgery for ovarian cancer?

Abstract: The treatment of ovarian cancer should be appropriate, since clinical and surgical decisions may affect the prognosis; the surgery must be performed by an expert oncological surgeon or gynecological oncologist, it's fundamental roles are cancer staging and cytoreduction. The concept of staging surgery in early stages has its justification in the fact that up to 11% of "early ovarian cancers" will have metastasis in different sites of the peritoneal cavity at the time of diagnosis. In advanced stages of epithel… Show more

“…In most of the patients a relatively simple surgery consisting of hysterectomy, bilateral salpingo-oophorectomy, infracolic omentectomy, limited excision of retroperitoneal node and segmental resection of intestine is sufficient. 30 A radical surgery involves more extensive procedures that are associated with greater peri-operative and post-operative complications. Aletti et al 31 published in 2006 a retrospective work on more than 194 patients aimed to analyze the effect of aggressive surgical effort on survival.…”

“…Ovarian cancer is a common gynecological malignancy that ranks rst in mortality among female reproductive system tumors due to the lack of speci c clinical manifestations in the early stages, and about 70-75% of patients are in the advanced stage at the time of diagnosis 20 .One of the reasons for the consistently high mortality rate in patients with ovarian cancer is that about 65% of patients are diagnosed in the International Federation of Obstetrics and Gynecology (FIGO) stage III or IV 21 . Ovarian cancer spreads mainly in an intraperitoneal manner, but retroperitoneal metastases are also common at all stages of ovarian cancer.…”
