How exclusive is exclusive breastfeeding? A comparison of data since birth with current status data

Abstract: Current status indicators based on a 24-hour period may be inadequate and even misleading for many purposes. We propose that in many studies an indicator called 'exclusive breastfeeding since birth' could be added.

“…Foi utilizado um questionário estruturado que versava sobre caracterís-ticas maternas e do bebê e sobre as várias ações da atenção integral à saúde da criança. Para a coleta dos dados referentes ao tipo de aleitamento materno praticado pela criança, foi utilizado o recordatório das últimas 24 horas -current status 21 22 , e aquelas que chegaram à unidade básica de saúde já no primeiro atendimento sem estarem sendo amamentadas, restando 1.029 crianças para análise.…”

“…In field studies on large samples it is difficult to distinguish exclusive breastfeeding from predominant or full breast-feeding 9 . Moreover, different methods and definitions are in use in breast-feeding epidemiology 2,20 . With our questionnaires we asked explicitly for non-milk fluid consumption and found differences of up to 17% between exclusive and full breast-feeding depending on age ( Table 4).…”

“…The reason for this very great difference was that mothers of infants who had not recorded having introduced water and tea in the 24-hour dietary recall, had offered these liquids to their infants in a certain moment of their lives. 1 Findings from the present study lead to a refl ection on the role of health services in the promotion of exclusive breastfeeding, once the level of implementation of both the mother-infant welcoming strategy and the Breastfeeding-Friendly Primary Care Initiative, developed by the city of Rio de Janeiro, is still low. The low implementation of these strategies is evidenced by the high proportion of mothers who had not received the mother-infant welcoming card at the moment of discharge from the maternity hospital, had not had the fi rst contact with the PHC unit in the infant's fi rst week of life, had not received guidance on breastfeeding in the fi rst visit to the PHC unit after child birth and had not received any guidance on breastfeeding in the PHC unit after child birth.…”
