History Repeats Itself

Abstract: Although many advances in stereotactic surgery appear to be of recent origin, there are precedents in the literature documenting the planting of those seeds that eventually grew into common procedures. Pallidotomy was the first stereotactic procedure in humans that used the Spiegel-Wycis apparatus in 1947. Other targets for Parkinson’s disease have their roots in the decade following that. Using stereotactic techniques to target solid or cystic lesions was introduced in the 1950’s, when either calcification or… Show more

“…Stereotactic ablative surgeries targeting the globus pallidus internal segment (GPi), the ventral intermediate thalamic nucleus, and the subthalamic nucleus (STN) have been a major tool for treating the symptoms of Parkinson's disease (PD) and other movement disorders for over 50 years (Gildenberg, 2003). Since the early days of ablation surgeries, electrical stimulation of these nuclei has been used to localize the target, leading to the observation that stimulation reduces tremor in patients.…”

“…98 According to Gildenberg, 37,38 who was a fellow of Spiegel and Wycis in the 1950s, intraoperative electrical stimulation was used from the very beginning as a mean of exploring the brain target prior to lesioning. Thus, from its very beginning, functional stereotactic neurosurgery was multidisciplinary, was directed at the treatment of psychiatric illness, and used electrical stimulation as a physiological means of assessing and corroborating the subcortical anatomical brain target.…”

“…However, stereotactic neurosurgery demands high accuracy during the procedure for reaching the target point in the intracranial space. The first attempts to create minimally invasive neurosurgery were made during the late 1940s and the early 1950s [1] with the development of stereotactic neurosurgery. In the early stage of stereotactic neurosurgery the imaging of the intracranial space was the most limiting factor.…”
