Histopathology Findings in Patients Undergoing Laparoscopic Sleeve Gastrectomy

Abstract: Most of the findings are non-significant and can be predicted if UGIE plus multiple biopsies is routinely included in the bariatric work-up with significant cost reduction. In those patients who had a significant finding prior to the surgery or intraoperatively, the pathological examination of the specimen is recommended.

“…Generally, pre-malignant lesions identified in LSG specimens across 11 studies varied from 4.1 to 33.2% [10,18,[36][37][38][39][40][41][42][43][44]. However, across these 11 abovementioned studies, there was zero actual malignancy reported, despite that these studies were undertaken in a wide variety of geographical areas worldwide with different risk of gastric cancers.…”

“…Thus, even with our 7.5% pre-malignant lesions identified in LSG specimens, and with the huge number of LSG procedures undertaken worldwide, such current lack of the evidence base renders us unable to extrapolate in any way the potential number of patients with pre-malignant lesions that would actually turn malignant or whether such numbers would be high and alarming or otherwise. Hence, there exists much debate regarding the role of routine histopathological examination of LSG specimens [10,18,[36][37][38][39][40][41][42][43][44], and some authors have suggested that histopathological examination of LSG specimens is to be done when intraoperative macroscopic pathology is observed [10,18,43]. Future research would benefit from addressing these points.…”

“…These patients need to be followed up strictly. Intestinal metaplasia can be detected at a rate of 0.2-5% and atrophic gastritis with a rate of 0.2-1.8% (Canil et al, 2018;Almazeedi et al, 2013). In our series, low-grade dysplasia was seen in 2 (0.5%) patients, intestinal metaplasia in 17 (4.1%), and atrophic gastritis in 1 (0.5%).…”

“…In the specimen, there were 200 (48.7) patients with signs of Helicobacter pylori-associated gastritis. Dysplasia can be seen at a rate of 0.06-0.2% in the histopathological examination of sleeve gastrectomy specimens (Canil et al, 2018;Safaan et al, 2017). These patients need to be followed up strictly.…”

“…Secondly, the current study reported positive significant correlation between BMI at time of enrollment and positivity for Hp. In support of such correlation, Hansen et al (29) detected a statistically significant association between preoperative Hp and significantly abnormal pathology detected in gastric sleeve excised specimens in obese patients and Canil et al (30) detected Hp in 24.6% of preoperative endoscopic gastric biopsy obtained from morbidly obese patients assigned for laparoscopic gastric sle eve surgery. Also, Šebunova et al (31) found about two-thirds of morbidly obese patients undergoing bariatric surgery are infected with Hp with a high prevalence of virulence genes and recommended treatment of this infection before surgery.…”
