Histopathological analysis of the mechanisms of intracranial hemorrhage complicating infective endocarditis.

Abstract: Background and Purpose:We conducted the present study to elucidate the pathological mechanisms leading to intracranial hemorrhage complicating infective endocarditis. Methods: Neurological, neuroradiological, and histopathological analyses were performed in 16 patients (one surgical and 15 autopsy cases), 12 men and four women 26-68 years of age, who had demonstrated central nervous system complications during the course of infective endocarditis. Results: Intracranial hemorrhage was found in all cases; parenchy… Show more

“…Presenting stroke subtypes included 11 with pure ischemic stroke, 2 with ICH, and 5 with a combination of ischemia and SAH. In patients with pure ischemic stroke, the median initial NIHSS was 14 (IQR [8][9][10][11][12][13][14].…”

“…3,7,11,13 Intracranial hemorrhage occurs in 5% of patients with infective endocarditis, 24 either due to hemorrhagic conversion of acute ischemic strokes or the formation of mycotic aneurysms. 16,22 Generally, treatment with intravenous thrombolysis up to 4.5 hours from symptom onset has become the standard of care in acute ischemic strokes. 10,17 To date, there has been much debate on how to treat ischemic embolic strokes from infective endocarditis.…”

“…Mycotic aneurysms are basically false aneurysms, so the wall is fragile and tight intrasaccular packing may cause rupture. 14) Additional parent artery occlusion enforces aneurysm exclusion from the circulation even if the intrasaccular packing is not very tight. The parent artery was already stenosed near the aneurysm neck in our case.…”
