ABSTRACT. The systemic arterial hypertension (SAH) currently a major public health problem, presents high medical and socioeconomic costs, and occurs concurrently with other diseases, of infectious origin or not. This study aimed to describe the sociodemographic profile of hypertensive patients with or without co-morbidities assisted in a health reference centre in a municipality of the northwest of the Paraná State, Southern Brazil. We gathered data from 250 medical records, concerning age, gender, race, marital status, educational level, professional duties, place of origin, reasons for seeking the services, occurrences of comorbidities, and duration of systemic arterial hypertension. SAH occurred more often in the age group of 60-69 years old. The female gender was significantly different from the male. We observed the predominance of married (60.0%), Caucasian (76.0%), with fundamental level of education (54.4%), housewife (39.6%), living in neighborhood towns (58.0%), born in other regions than the Southern Brazil and have seek the service mainly for reasons not related to hypertension (p < 0.05). Cardiopathy of diverse etiology, Diabetes mellitus, Chagas disease and acute myocardial heart attack were identified in 98 patients (39.2%). Long-standing SAH was observed on 65.2% of the patients. The knowledge of the profile of the hypertensives, and principal associated diseases allows directing health actions in order to optimize resources, and make effective control of blood pressure and co-morbidities, including alternative intervention approaches to increase life expectation of patients.Keywords: arterial hypertension, comorbidities, sociodemographic profile, Paraná State, Southern Brazil.Hipertensão arterial: perfil sociodemográfico e comorbidades de pacientes da região Noroeste do Paraná, Sul do Brasil RESUMO. A hipertensão arterial sistêmica (HAS), atualmente um dos principais problemas de saúde pública, apresenta custos médicos e socioeconômicos elevados e ocorre concomitantemente com outras doenças, de origem infecciosa ou não. Este trabalho teve como objetivo descrever o perfil sociodemográfico e comorbidades de pacientes hipertensos atendidos em um centro de referência em saúde em município da região Noroeste do Paraná, Sul do Brasil. De 250 prontuários foram coletados dados referentes à idade, sexo, raça, estado civil, escolaridade, atividade profissional, procedência, naturalidade, motivo da procura pelo serviço, ocorrência de comorbidades e tempo da HAS.A HAS teve maior frequência na faixa etária de 60-69 anos. O gênero feminino foi significativamente diferente do masculino. objective: Based on the analysis of indicators which express the rationale of the actions of the program, we developed a crosssectional study to assess the performance of a Family Healthcare Team as regards the care provided to individuals with arterial hypertension living in a small municipality. Methods:The study carried out was based on the analysis of the records (Record A) of SIAB (Primary Care Information System) of 418 individuals suffering from arterial hypertension, of 351 records stored in the primary healthcare center, and of 376 interviews with individuals living in the area under the team's care, in the year 2003. The following variables were studied: gender, age, family income, housing conditions, health services used, presence of diabetes mellitus, coverage and concentration of care. results: Women above 60 years of age prevailed. They were 65.7% of the cases. The arterial hypertension prevalence rate was 18%, and was different for both genders: for women it was 22.3%. The coverage was 64.1% for appointments with doctors, 32.4% for appointments with nurses, and 36.4% for home visits. The information on files revealed that 25.8% of hypertensive individuals did not receive care in 2003 and 52.7% did not receive care in the last semester. The concentration of care was below the recommendation. conclusion:The information showed that the organization of health care does not reflect the rationale of health surveillance in that services focus mostly on meeting spontaneous demand, which in turn reflects unpreparedness as to the use of information systems for the planning and evaluation of actions.

Efficiency and costless of a long-term physical exercise program to non-medicated hypertensive males

Hipertension is a highly prevalent disease that often goes indetected and is associated with other comorbities and risk factors that create high costs and overhead. Lowering of blood pressure and prevention of hypertension is in first instance preferable by lifestyle changes with aerobic exercise being an integral component. However it has been neglected frequently by heatlh care programs that has costly medication actions instead. To investigate the aerobictraining/detraining effects on blood pressure (BP) efficiency and costs the study involved 80 adults, overweight men who were not taking antihypertensive medications or dieting, and were accomplished to all intervention tasks. Based on 112 Roberto Carlos Burini et al.their initial BP the subjects were assigned for either a normal BP (NG, n=31, SBP 123.7 ± 8.3/DBP 79.9 ± 6.5 mmHg, BMI 28.3 ± 3.7 kg/m 2 ) or high BP (HG, n=49, SBP 146.5 ± 11.2/DBP 94.3 ± 7.5 mmHg, BMI 29.4 ± 3.7 kg/m 2 ) groups. The intervention phase was a two (NG)-three (HG) year period in which all the subjects followed an 8 month/year supervisioned exercise training followed immediately by a 4 mo./yr rest (detraining). The aerobic (80% of maximum heart rate) training occurred 3-5 times a week for 30 minutes walking/jogging, in addition to 30 min warm up and cool down. BP measurements were taken before and after the exercise sessions (LISC program). No dietary changes were recommended during the exercise intervention. Descriptive and mean values comparisons were undertaken among moments of intervention and between groups for p < 0.05. The LISC protocol gradually decreased the BP at rest. In the HG the 2-8 mo. average reductions in SBP and DBP varied from 6.2 to 11.9 mmHg and from 3.1 to 10.2 mmHg respectively. In the following years the BP decreased 8.0 (year 2) and 7.7 mmHg (year 3) for SBP and 7.2 and 4.9 mmHg for DBP. The normal BP was restored yearly after the 8 mo. training. The BP responses also occurred in NG, confirming that physical exercise is an effective approach to preventing hypertension. It is noteworthy that the BP reductions occurred in a stable diet and body weight situations. The 4 mo. detraining period resulted in BP increase nearly back to their baseline levels. With the found efficiency of 31.6% in normalizing high BP (affecting 28.5% of the Brazilian adults at an estimated cost of US$ 1.18 billion a month) this 8 mo LISC (at a professional cost of US$ 6000/8mo) would result in an estimated save of US$ 373.3 million (spent with medication drugs) if the LISC would be replicated nationwide. In conclusion the present LISC as a tool for high BP normalization demonstrated to be feasible, efficient and costless (in comparison with drugs) highly recommendable for replication in other similar communities.
