High level of medication regimen complexity index correlate with worse quality of life in people living with HIV

Abstract: Objectives. People living with HIV (PLWHIV) have now a near-normal life expectancy and thus, a higher risk of polypharmacy. The main objective was to assess the correlation between medication regimen complexity index (MRCI) and quality of life (EQ-5D) and health utilities among PLWHIV patients on ART. Patients and methods. Observational prospective single-center study including adult PLWHIV on ART from January to March-2020 attended at hospital pharmacy outpatient service according to a Capacity-Motivation-Opp… Show more

“…Notably, as shown in the adjusted analysis, very complex regimens were associated with poorer adherence, regardless of the number of drugs, since schemes with the same amount of drugs may present different degrees of complexity, as assessed by the MRCI, due to other aspects evaluated by the tool 23 . It should be noted, for example, that carelessness with the administration schedules of medications has been mentioned as an essential factor that impacts treatment adherence 19, 24 .…”

“…Analysis using the t-student test showed a statistically significant relationship between MRCI score and the sum of the STOPP-Beers criteria with increasing age ( p < 0.05) The generalisability of the study findings is reduced by the single-centre study design and small sample size. Moreover, the STOPP-Beers criteria were validated to non-HIV patients 65 years or older Contreras-Macias et al, 2021 (Spain) [ 70 ] n = 428 M = 50 years (SD = 10.9) Outpatient Data were collected using the Capacity-Motivation-Opportunity pharmaceutical care model at routine follow-up appointments Medication Regimen Complexity Index (MRCI) and the EQ-5D-5L questionnaire Polypharmacy was identified in 25.9% (111) of participants, with 5.4% (23) being on 11 or more medications. A negative correlation between ED-5D and MRCI scores was identified ( p = 0.0002).…”

“…The presence of one or more co-morbidities also increases the risk of falls in PLWH [ 50 ]. Among older PLWH, poor HRQOL is associated with co-morbidities, especially AIDS-defining illnesses, longer time since ART initiation, loneliness and a lower level of education [ 26 , 54 , 62 , 65 , 68 70 ]. A study focusing on women living with HIV showed that physical HRQOL was lower in older participants than in younger age groups [ 69 ].…”

“…In our study, almost half of the patients present a high complexity index, with a median of 11.0 points. There are other published studies, such as the one by Contreras et al [ 34 ], where a lower mean complexity index (6.9 points), with 20.6% of patients with a high complexity index. This difference may be due to the fact that the study was carried out in HIV patients older than 18 years.…”
