High grade prostate intraepithelial neoplasia (PIN) is a PSA-independent risk factor for prostate cancer in African American men: Results from a pilot study

Abstract: African American men (AAM) demonstrate increased prostate cancer incidence and mortality rates. We investigated known prostate cancer risk factors in AAM. Prostate specific antigen (PSA) and diagnosis of high grade prostatic intraepithelial neoplasia (PIN) were significant prostate cancer predictors. However, even including AAM with low PSA (<4ng/ml), those with PIN had significantly elevated risk, compared to men without PIN (83.3% vs 6.9%, p<0.0001). In AAM diagnosed with PIN, PSA level was no longer signifi… Show more

“…The present study was based on 35 SNPs that can potentially modify the risk of cancer. A literature research revealed that only 10 of these 35 SNPs were described in studies of African or Afro-American populations (Mechanic et al, 2007;Chornokur et al, 2013a;Nikolić et al, 2014;Oh et al, 2017a;Oussalah et al, 2017;Tong et al, 2018;Jones et al, 2019;Sagna et al, 2019;Song et al, 2019;Kamiza et al, 2020), suggesting that most association studies including these SNPs focused on European and Asian populations. The present results showed that a high number of individuals were homozygous for risk alleles with Native American and African ancestry that may modify the risk of cancer.…”

“…Patients within the non-metastatic prostate cancer cohort were also more likely to have comorbidities of high-grade PIN and colon polyps. Prior diagnosis of high-grade PIN is highly predictive of prostate cancer diagnosis [20]. Familial cancer link, inflammation, and mutations leading to reduction in phosphatase and tensin homolog (PTEN) expression have been implicated in the development of benign and malignant gastrointestinal lesions and in prostate cancer development [2123].…”

“…33 Recent results from the largest prospective randomized trial of its kind with 1467 untreated men with isolated PIN and central pathology review revealed a cancer yield of 34.7% in repeat biopsies obtained from 6 to 36 months, underscoring the significance of this microscopic finding. 10 If all procedures fail to identify coexistent carcinoma, close surveillance and follow-up are indicated. 10 If all procedures fail to identify coexistent carcinoma, close surveillance and follow-up are indicated.…”
