High-Grade Neuroendocrine Carcinoma of the Cervix

Abstract: Neuroendocrine tumor of the uterine cervix is a rare and aggressive malignancy. Despite controversial, multimodal treatment methods, prognosis and treatment outcomes are worst in advanced stages. We report an early-stage case treated with the multimodal approach. The nomenclature proposed for this tumor type in 1997 by the College of American Pathologists still has some points of discussion. As in our case some of the tumors cannot be defined exactly into small or large cell types, and this causes confusion at… Show more

“…The exact dosage and therapy duration of this scheme, however, varied considerably in the published studies. For example, Baykal et al used cisplatin 80 mg/m 2 on day 1 together with etoposide 120 mg/m 2 on days 1, 2, and 3 in a 21 day cycle [ 26 ]. Intaraphet et al used cisplatin 75 mg/m 2 and etoposide 100 mg/m 2 every 3 weeks [ 59 ].…”

“…Although the incidence of this kind of tumor is low, it is prone to lymphatic and hematogenous metastasis in the early stage, and its prognosis is significantly worse than that of cervical squamous cell carcinoma and adenocarcinoma in the same period [30]. e prognostic factors for NEC patients have not yet been determined, but most studies indicated that the main factors affecting the prognosis of NEC patients include clinical stage, tumor size, lymph node metastasis, and the efficacy of treatment; thus, the early and precise diagnosis of cervical NEC is quite significant to improve the prognosis [31]. However, the rate of misdiagnosis for NEC is considerably high even for specialist pathologists [31], raising the question whether a computer-assisted pathological diagnosis may be helpful to improve the accuracy of diagnosis [31].…”

Nevertheless, the automatic detection of NEC lesions by computer on WSI is quite challenging due to the extremely small amount of data and the unavailability of precisely labeled data due to the low prevalence of the tumor and the huge labor consumption of manual annotation.…”

“…However, SCNEC is an aggressive type of cancer that develops distant metastases even in its early stages [4,5]. Lymph node involvement and lymph vascular space invasion are thus present relatively early on in the progression of SCNEC and are related to early relapse due to hematogenous dissemination [6,7].…”
