Global Distribution of Hepatitis D Virus Genotypes: A Systematic Review

Context: Hepatitis D virus (HDV) infection, as the main coinfection in patients with chronic hepatitis B, leads to progressive liver disease. Elucidating the global distribution of HDV genotypes may be beneficial for the development of HDV vaccines and antiviral agents. Objectives: Through this systematic review, we aimed to present a clear picture of HDV genotype dispersal at the global and regional levels. Methods: A systematic search was performed in Scopus, PubMed, and Web of Science using the relevant keywords of hepatitis D and HDV genotype. The old HDV genotype classification (HDV I, II, and III) and African HDV genotypes (HDV-5,-6,-7, and-8) were used for showing the HDV genetic dispersion. The data of the country-level distribution of HDV genotypes were translated to the regional and global distributions of HDV genotypes. Results: Among all 1,318 unique titles, 71 studies were screened in the qualitative synthesis consisting of 77 records from 33 countries (sampling locations). In Africa, the most common genotype was HDV I, followed by African HDV genotypes. In Asia, the most frequent genotype was HDV II, followed by HDV I. In Europe and Oceania, the most common HDV genotype was HDV I, followed by HDV II and African genotypes. In the Middle East and North America, the most frequent HDV genotype was HDV I. In South America, the most common HDV genotype was HDV III, followed by HDV I and African genotypes. Conclusions: We found HDV I is distributed globally. Other HDV genotypes are observed regionally: HDV II mainly in East Asia, HDV III exclusively in South America, and African genotypes mainly in West Africa. PROSPERO #: CRD42018097296.

Objective: Hepatitis delta virus (HDV) is the smallest known virus to infect humans. HDV, a defective ribonucleic acid (RNA) virus, is also the most important cause of liver-related mortality in patients with hepatitis B. Fifteen-20 million people worldwide are estimated to be infected with HDV. In this study, we aimed to determine the epidemiological and clinical manifestations of HDV infection and to asses the data on clinical and laboratory findings, treatments, serologies and biopsy results in patients with HDV infection. Materials and Methods: The records of patients with HDV infection who attended the infectious diseases and clinical microbiology clinics at Atatürk University Medical Faculty Hospital between 2008 and 2013 were retrospectively analyzed. Antigen and antibody results were investigated using enzyme-linked immunosorbent assay (ELISA) (Dia. Pro Diagnostic Bioprobes Srl, Milan, Italy). Determination of hepatitis B virus (HBV) DNA values and HDV RNA (Qiagen © , Germany) was performed using real-time polymerase chain reaction. Liver biopsies were performed and the specimens were assessed using Knodell's scoring system. Results: Two thousand five hundred forty hepatitis B surface antigen (HBsAg)-positive patients were included in the study. One hundred and three (4.05%) patients (35 female-34%-and 68 male-66%-) aged between 19 and 70 years (median: 49 years) were HDV antibody (anti HDV)-positive. Cirrhosis developed in 10 (9.7%) patients. Sixty (58.3%) patients were anti HBe-positive, while 34 (33%) were hepatitis B e antigen (HBeAg) and anti-hepatitis B e (anti-HBe) negative. Conclusion: HDV should be investigated in patients with hepatitis B due to its association with the increased risk of progression to liver cirrhosis (LC) and hepatocellular carcinoma, as well as mortality rates. Hepatitis D is still a significant problem in Turkey and especially in our region. Nationwide hepatitis B vaccination programs and frequent screening of blood specimens will prevent hepatitis D.

Background: Patients coinfected with HBV and hepatitis D virus (HDV) have a greater risk of HCC and cirrhosis. The current study was undertaken to assess HDV genotype distribution and determine clinical characteristics of hepatitis delta virus (HDV) among HBsAg positive individuals in Shanghai.Method: This retrospective study involved 225 serum samples from HBsAg positive hospitalized patients from October 2010 to April 2013. HDV-specific RT-nested PCR was used to amplify HDV RNA. HDV genotypes were characterized by Next-generation sequencing (NGS), followed by phylogenetic analyses. HDV/HBV co-infected patients and HBV mono-infected patients were compared clinically and virologically.Results: Out of the 225 HBsAg-positive serum samples with elevated transaminases, HDV-RNA was identified in 11 (4.9%) HBsAg positive patients. The HBV loads in the HDV positive group were significantly lower than the HDV negative HBV-infected patients. The aminotransferase enzymes were significantly higher in HDV/HBV co-infected compared to HDV negative patients (P<0.05). Phylogenetic analyses indicated that HDV-2 genotype being the predominant genotype, other HDV genotypes were not observed. HDV/HBV patients were significantly associated with a rather unfavourable clinical outcomeConclusion： In summary, our study showed that the prevalence of HDV infection in patients with elevated transaminases is not low and the predominance of HDV genotype 2 infection in Shanghai. This finding helps us to better understand the correlation of HDV/HBV co-infection. Moreover, Next-generation sequencing (NGS) technologies provide a rapid, precise method for generating HDV genomes to define infecting genotypes.
