Hepatosplenic and Muscular Sarcoidosis: Characterization with MR Imaging

Abstract: Sarcoidosis is a multisystem disorder of unknown etiology that involves multiple organs. Computed tomography is theˆrst-line imaging modality for diagnosing sarcoidosis because of its capacity to detect hilar lymphadenopathy and pulmonary lesions. Magnetic resonance (MR) imaging provides good soft tissue contrast that is useful for detecting sarcoidosis in some body parts, including skeletal muscle. Signal intensity on pre-and postcontrast T 1 -and T 2 -weighted imaging may re‰ect disease activity and the path… Show more

“…Osteoporosis and soft tissue swelling may be seen on X-ray (14). Muscular sarcoidosis is reported in 50% to 80% of patients but muscle lesions were observed in less than 0.5% of cases (5, 11, 13, 15); however, symptoms occur in less than 0.5% of patients (5, 7) who may present with local pain and tenderness, muscle cramps, palpable nodules, or muscle pseudohypertrophy and contracture (8, 11, 13). Since the muscle enzymes such as creatine kinase, aldolase, and myoglobin are elevated in such patients, other diseases that are associated with elevated muscle enzymes (e.g.…”

“…Splenic involvement is confirmed in up to 60% of patients by biopsy. 15 Although liver was considered as an easily accessible organ for an initial biopsy, we were not able to confirm the diagnosis of sarcoidosis with the findings of liver biopsy.…”

“…L'aspect classique est celui d'une hépato-splénomégalie hétérogène avec des nodules multiples et infra-centimétriques [14,20,22]. Les larges nodules de régénération centraux ainsi que les zones d'atrophie périphériques du parenchyme hépatique sont les aspects les plus suggestifs du diagnostic [23].…”
