Background:Sarcoidosis is a multisystem disease affecting different organs with different frequency rates depending on geographical location. Musculoskeletal abnormalities includes osseous lesions (small and large bone sarcoidosis), sarcoidal arthropathy, and sarcoidal myopathy. Musculoskeletal involvement is reported in a significant number of patients.Objectives:This study aimed to determine the prevalence and clinical picture of musculoskeletal sarcoidosis in Iranian cohort of patients with sarcoidosis.Patients and Methods:We designed a descriptive cross-sectional study including 30 patients with sarcoidosis who had hospitalized in the Rheumatology Department at Ghaem Hospital, Mashhad, Iran. The patients were evaluated for musculoskeletal symptoms using history, physical examination, and paraclinical data.Results:Of the 30 studied patients, 24 were female (80%) and six were male (20%). The mean age at diagnosis was 38 years. Sarcoidal arthropathy (arthritis and periarthritis) was observed in 26 patients (86.6%). Furthermore, the initial presentation was associated with joint symptoms in 19 cases (63.3%); acute arthritis developed in 17 (65%) while bone and muscle involvements each occurred in 2 (6.6%).Conclusions:Sarcoidosis is a common disease in women aged 20 to 40 years. The most common involved joint were consecutively ankles, knees, and wrists, reaching a accumulated frequency of 86.6%; however, bone and muscle involvements were uncommon.

Sarcoidosis, a multisystem disorder of unknown etiology that involves multiple organs, is rare in children. The true incidence and prevalence of childhood sarcoidosis is unknown. As in adults, many children with sarcoidosis may be asymptomatic; the disease may remain undiagnosed. A complete and systematic evaluation of the patient is essential for the sarcoidosis diagnosis in children. Here, we describe a case of 12-year-old female who presented with 2 years history of uveitis and hepatosplenomegaly. A chest computerized tomography revealed scattered peripheral pulmonary nodules and bilateral hiliar lymphadenopathy. Bone marrow aspiration and liver biopsy were not diagnostic. A lung biopsy showed non-necrotizing epithelioid cell granulomas. She was diagnosed with sarcoidosis according to demonstration of granulomatous inflammation and the exclusion of confusable entities.

Les localisations hépatospléniques symptomatiques inaugurales de sarcoïdose sont exceptionnelles. Nous rapportons l'observation d'une patiente de 47 ans, non tarée qui fut admise pour bilan étiologique d'un ictère cutanéo-muqueux d'aggravation progressive associé à un prurit géné-ralisé et évoluant depuis un mois. L'examen physique trouvait en plus de l'ictère cutanéo-muqueux franc, une hépato-splénomégalie ferme, irrégulière et indolore. La biologie montrait une cholestase et une cytolyse hépatique modérée sans signes d'insuffisance hépatocellulaire. L'imagerie médicale notait une hépato-splénomégalie hétérogène sans lésions focales ni adénopathies profondes associées. La biopsie hépatique montrait une hépatite granulomateuse non nécrosante et le bilan étiologique de cette granulomatose concluait à une sarcoïdose. Sous corticothérapie systémique l'évolution était favorable cliniquement, biologiquement et radiologiquement.Une meilleure connaissance de ces localisations ainsi que leur détection précoce se trouvent justifiées car influencent la stratégie thérapeutique et conditionnent le pronostic de cette granulomatose.Mots clés Sarcoïdose · Hépato-splénomégalie · Granulomatose · Imagerie · Cholestase Abstract Inaugural symptomatic hepatosplenic locations of sarcoidosis are exceptional. We report the case of a 47 years old patient without medical history who was explored for progressive worsening mucocutaneous jaundice, associated with generalized pruritus, and lasting for one month. Physical examination revealed mucocutaneous jaundice associated to an irregular and painless hepatosplenomegaly. Biology showed cholestasis and moderate hepatic cytolysis without signs of liver failure. Medical imaging noted heterogeneous hepatosplenomegaly without focal lesions or deep lymph nodes. Liver biopsy showed necrotizing granulomatous hepatitis and etiological investigation for this granulomatosis concluded to systemic sarcoidosis. Under corticosteroids the evolution was favorable clinically, biologically and radiologically.A better understanding of these locations as well as their early detection are justified because they influence the therapeutic strategy and determine the prognosis of this granulomatosis.
