Hepatitis B viral factors and treatment responses in chronic hepatitis B

Abstract: Baseline and on-treatment hepatitis B viral factors are reported to affect treatment responses. A lower baseline hepatitis B virus (HBV) DNA level is a strong predictor of the response to antiviral therapy. HBV genotype A/B patients have better responses to interferon-based therapy than those with genotypes C/D. Regarding the association of HBV mutants with responses to antiviral therapy, current evidence is limited. On-treatment viral suppression is the most important predictor of response to nucleoside analo… Show more

“…Usually, genotype C virus is associated with more severe liver diseases than is genotype B [33]. In addition, patients with HBV genotypes C and D have lower response rates to treatment with IFN-α compared to those with genotypes A and B [34]. Among the 48 HBV strains isolated from the present study, 33 strains belonged to genotype C. Since genotype C HBV is very closely associated with severity, development of severe liver diseases, and antiviral therapy, the larger numbers of the schizophrenia patients and long-time follow-up studies are required.…”

“…Low HBV DNA levels and high ALT levels prior to NA therapy are considered to be predictors of HBeAg seroconversion . In the GLOBE study, on‐treatment monitoring was evaluated in 1367 patients with CHB .…”

“…Optimization therapy according to baseline characteristics, such as gender, duration of infection, HBeAg level, HBV DNA level, and ALT level, has been used as a strategy to predict the virological response to NAs . Nevertheless, to date, few studies have assessed predictors of response to NAs . Patients with optimal baseline characteristics (so‐called super‐responders) are more likely to achieve favorable outcomes after treatment with telbivudine .…”

“…However, the optimal timing and strategy of antiviral therapy for chronic HBV infection are somewhat difficult. With recent advances in molecular investigations, hepatitis B e antigen (HBeAg), serum alanine aminotransferase, HBV DNA, and HBV genotype, which are associated with the response to antiviral therapy, have been identified in patients with chronic hepatitis B (CHB) [5,6]. These biomarkers predict poor clinical outcomes in patients with CHB and help to individualize their antiviral therapy.…”

“…These biomarkers predict poor clinical outcomes in patients with CHB and help to individualize their antiviral therapy. Several studies have demonstrated that a lower baseline HBV DNA level predicts HBeAg loss or HBeAg seroconversion as well as viral suppression in patients with CHB receiving antiviral therapy [5]. Baseline and on-treatment response of HBV DNA level are well-established predictors of treatment outcomes in patients with CHB on antiviral therapy.…”
