Hepatitis B Surface Antigen Should Not Be the Only Sought Marker to Distinguish Blood Donors towards Hepatitis B Virus Infection in High Prevalence Area

Abstract: Since its discovery by Blumberg in 1965, the hepatitis B surface antigen (HBsAg) is used as the fingerprint of hepatitis B infection. Occult hepatitis B infection (OBI) is defined by a viral replication (DNA detectable) in the absence of HBsAg. Burkina Faso is a high endemic area where the prevalence is higher than 14%. At the National Center for Blood Transfusion (NCBT) of Ouagadougou, HBsAg is the only sought marker used to distinguish donors towards Hepatitis B Virus (HBV). Acceptation of blood donation is … Show more

“…The latter is lower than that reported among HIV-positive patients from Ivory Coast in 2010 and from Sudan in 2014, and among blood donors from Burkina Faso in 2016; 10%, 15%, and 32.8% respectively. 17 , 21 , 22 Nevertheless, our prevalence was similar to that of 6.25% reported among Egyptian blood donors in 2010. 19 However, it was higher than 0.5% reported among regular blood donors in Southeast Nigeria.…”

“…This prevalence is lower than 44.0% reported in HBsAg-negative blood donors in Burkina Faso. 17 However, it is higher than 7.8% and 16.6% reported in HBsAg-negative blood donors in Egypt. 19 , 20 These differences could be explained by the size and type of study population but also by endemicity for HBV.…”

“…Added to this is the high prevalence of occult hepatitis B reported in several studies in Burkina Faso. 28 , 29 Occult hepatitis B is characterized by the presence of HBV DNA in the serum of a patient who is screened for HBsAg by the usual serological tests. 30 Although our study did not allow us to detect residual cases associated with HIV and HCV, the number of donations detected positive HBV by PCR confirms that PCR is more sensitive than ELISA.…”

“…Occult HBV infection is transmissible through blood transfusion in the event of biological qualification of donations without the use of the polymerase chain reaction (PCR) assay, and through organ transplants. The prevalence of occult HBV infection in Burkina Faso varies according to studies; Birama et al, (10) reported a prevalence rate of 7.3% in 2018 in the population of Ouagadougou while Somda et al, (11) reported a seroprevalence rate of 32.8% among blood donors in 2016.…”
