Hepatic schistosomiasis japonica identified by CT.

Abstract: We reviewed the CT examinations of 17 patients who had hepatic schistosomiasis japonica and noted peculiar hepatic calcifications in 15 cases and a notch on the hepatic surface in six. A hepatocellular carcinoma was found in six patients. The calcifications and notch appeared to indicate the degree of hepatic fibrosis but did not correlate with cirrhotic deformation.

“…This lesion is thought to be due to septal calcification of schistosomal ova. 60 In the diagnosis of hepatosplenic schistosomiasis using MRI, the most frequent findings are accentuation of periportal signal in T2-weighted sequences, and a hypointense signal in relation to the normal liver parenchyma in T1-weighted sequences with fat suppression. The periportal signal is accentuated on T1-weighted sequences following contrast administration.…”

“…Typical findings of schistosomiasis on US and CT are a network pattern, high-density bands and spotty or linear high-density lesions in the liver associated with calcification of deposited eggs, as seen in this case. 4,5 Laparoscopy showed typical findings; that is, multiple whitish markings and irregular, groove-like septa which are seen in advanced cases. In severe cases, block-like formations separated by groove-like depressions are reported.…”

“…Compared with ultrasound, the CT scan provides accurate diagnosis of ectopic forms of schistosomiasis involving the central nervous system, pulmonary, and other organ sites. The network pattern seen in the liver in schistosomiasis japonica has been delineated by CT as septal calcification [38,39]. The septal calcification of schistosomal ova in S. japonica has been thought to be responsible for the “turtleback” or “tortoiseshell” appearance of the hepatic lesion [39].…”
