Hepatic Fibrosis: Prospective Comparison of MR Elastography and US Shear-Wave Elastography for Evaluation

Abstract: MR elastography and SWE shear-wave elastography showed moderate correlation and similar diagnostic performance in the diagnosis of HF hepatic fibrosis of stage F2 or greater; however, MR elastography yielded more reliable LS liver stiffness measurements than did SWE shear-wave elastography .

“…MRE can be easily added to standard abdominal MRI protocols, promising value added in staging liver cirrhosis. One study showed that MRE and SSI results moderately correlated with liver cirrhosis values, though MRE measurements tended to be more reliable than US elastography [56] . MRE has many advantages: (1) it can exam the whole liver, with a lower sampling error than with a biopsy or other imaging modalities; (2) good diagnostic accuracy; and (3) the results are not influenced by hepatic steatosis, obesity, and ascites.…”

“…54 -66 However, only a limited number of clinical studies have compared the discriminative abilities of MRE and ultrasound-based elastography for staging hepatic fibrosis. [58][59][60]66 Two of those revealed the superior ability of MRE to Fibroscan ÎŒ for distinguishing stages of hepatic fibrosis, 58,60 and the others revealed the similar diagnostic performance of MRE, Fibroscan ÎŒ , and SWE for staging hepatic fibrosis. 59,66 Consequently, it might be concluded that the discriminative ability of MRE for staging hepatic fibrosis is equivalent or superior to that of ultrasound-based elastography.…”

“…Point shear-wave elastrography is similar to FibroScan but requires more expertise, is less well validated, and is not a point of care test ( 17,18,30 ). 2D MRE has the advantage of examining a larger area of the liver, is operator independent, and has excellent accuracy (17)(18)(19)29,31 ). Given the heterogeneity of hepatic fi brosis, particularly in more severe stages, MR elastography can more fully evaluate fi brosis as compared with the limited sampling of ultrasound FibroScan and even liver biopsy.…”
