Hepatic angiosarcoma: Aspiration biopsy cytology and immunocytochemical contribution

Abstract: In the absence of previously documented sarcoma, the initial diagnosis of angiosarcoma (AS) on fine‐needle aspiration (FNA) biopsy of the liver is difficult. However, awareness of its occurrence and the assistance of immunocytochemical stains may aid in arriving at the correct diagnosis. In this paper, a 59‐yr‐old smoker and alcoholic woman presented after a syncopal episode and was found to have a palpable right abdominal mass. An abdominal CT scan confirmed multiple centrally necrotic liver masses, from whic… Show more

“…These nucleoli had a distinctive bar or bullet shape and were 5 or more times longer than they were wide. The nucleoli in angiosarcoma have been previously described as absent, small, or prominent in previous studies . Pencillate nucleoli have been described in histologic resections of angiosarcoma and are highly suggestive of angiosarcoma.…”

“…On immunohistochemical staining, the tumor cells were found to be positive for cluster of differentiation (CD)31 which originated from the vascular endothelium, supporting a diagnosis of hepatic angiosarcoma ( Fig. 4D) (9). Partial areas of the tumor showed necrosis during TAE.…”

“…The diagnosis of HAS can be very difficult despite the introduction of nuclear magnetic imaging [23]. As liver needle biopsy does not many times allow making a correct diagnosis, adequate tissue sampling (using excision biopsy) is of utmost importance to avoid futile surgery.…”
