Heparin induced thrombocytopenia

Abstract

ÖZET Heparine bağlı trombositopeni (HBT) heparin tedavisinin nadir fakat ciddi bir protrombotik yan etkisidir. Bu durum platelet faktör 4 ve heparin komplekslerine karşı trombosit aktive edici antikorlar tarafından indüklenir. Tanı hastalık olasılığının klinik değerlendirmesi ve laboratuvar test-lerine dayanmaktadır. Erken tanı, heparin kullanımının kesilmesi ve müteakiben alternatif antikoagülan ilaçlar ile tedavi trombüs oluşumu, ekstremite amputasyonu ve ölüm gibi ciddi komplikasyonların önlenmesi için önemli adımlardır. Bu derlemede, HBT'nin klinik özelliklerini ta-nımlamak ve onun yönetimi için mevcut verileri özetleme-yi amaçladık. Anahtar sözcükler: Heparin, trombositopeni, kardiyak cerrahi. ABSTRACT Heparin-induced thrombocytopenia (HIT) is a rare but serious prothrombotic adverse effect of heparin treatment. It is induced by platelet-activating antibodies against complexes of platelet factor 4 and heparin. Diagnosis rests on a clinical assessment of disease probability and laboratory testing. Prompt diagnosis of HIT, discontinuation of heparin use, and subsequent treatment with alternative anticoagulant drugs are essential steps to prevent serious complications such as thrombus formation, limb amputation , and death. In this review, we describe the clinical features of HIT and to summarize the data available for its management.
