Hemorrhagic onset of spinal angiolipoma

Abstract: Spinal angiolipomas are rare benign tumors that generally induce slow progressive cord compression. Here, the authors describe a case of sudden-onset palsy of the lower extremities caused by hemorrhagic spinal angiolipoma. An emergent laminectomy was performed to achieve total lesion removal. Follow-up examinations indicated neurological improvement and the absence of recurrence.

“…22,26,27 Critical onset or aggravated neurological symptoms occur if the lesion grows rapidly because of tumor thrombosis and/or hemorrhage. [28][29][30][31] X-rays of the spine often show negative results in patients with SALs, but might show erosion of the walls of the bodies and pedicles as a result of infiltrating tumors. 8,32 Myelography was performed in some cases and showed a characteristic posterior extradural spinal lesion with partial or complete filling defect.…”

“…Likewise, a rapid neurological deterioration was observed in the current case that is why we had initially diagnosed it as spinal epidural hemorrhage. Sudden onset or worsening of neurological findings occurred when there is a rapid increase in tumor size of angiolipomas due to intratumoral thrombosis, massive acute hemorrhage, or steal phenomenon [ 5 , 7 9 ]. Vigorous exercise can contribute to augment blood flow size in the tumor to produce epidural bleeding [ 7 ].…”

“…Most angiolipomas are slow-growing and present with gradually progressing radicular symptoms. In patients with intratumoral hemorrhage, the symptoms can include acute-onset myelopathy 2) . Our patient had bleeding around the mass, and the development of lower extremity weakness was relatively subacute.…”
