The aim of this study was to analyze the impact of various donor and machine parameters on PLT yield in 127 PLT apheresis procedures, to optimize PLT yield achieving clinical and economic advantages. One hundred and twenty-seven apheresis procedures were analyzed. Age, gender, volume processed, Hb, and PLT precounts were included as donor predicting variables. AC infusion rate, processing time, and plasma volume collected with PLTs were assessed as machine parameters. We evaluated the post-transfusion effectiveness in 23 patients with thrombocytopenia, studying the effect of PLT dose, ABO group, and PLT storage time. Females gave higher yields, compared to males, P<0.01. PLT yield correlated positively with PLT precount (r=0.512), and TBV (r=0.404), and negatively with donor preapheresis Hb (r=-0.306). Processing time and AC infusion rate had a positive impact on PLT yield. Post-apheresis decrease in PLT count was 53.6+/-26.3x10(11). Donors with Hb>or=12 g/dl, donated safely. Most of the complications were citrate related (13.4% of all procedures). PLT increments in transfused patients correlated positively with the number of units transfused (r=0.41), and negatively with PLT storage days (r=-0.342). PLT increments in patients receiving ABO-compatible PLTs were 75% higher, compared to the increments in patients receiving incompatible PLTs. PLT count and volume processed were the main predictors of PLT yield. Increasing the processing time, the AC infusion rate, or the volume of plasma obtained with PLTs can increase PLT yields. High PLT dose, short storage time, as well as ABO compatibility should be considered during PLT transfusion.

Introduction:Platelet concentrate (PC) remains one of the most important support measures in thrombocytopenic patients. An efficient cell separator is a prerequisite for an optimally functioning apheresis setup. Donor blood count may undergo a temporary reduction after the procedure.Aim:The aim was to find the extent of reduction in donor blood count (hemoglobin, hematocrit, white blood cell, and platelet) after plateletpheresis and to evaluate the cell separator for collection efficiency, processing time, and leukoreduction.Study Design and Methods:Two hundred and thirty seven procedures performed on the Amicus (N = 121), Fenwal CS-3000 Plus (N = 50) and Cobe spectra (N = 66) in a one year period were evaluated. The procedures performed on the continuous flow centrifugation (CFC) cell separators and donor blood counts (pre and post donation) done were included in the study.Results:The percent reduction in hemoglobin (HB), hematocrit (HCT), white blood cell (WBC) and platelet count ((PLT ct) was 2.9, 3.1, 9, 30.7 (Mean, N = 237) respectively after the procedure. The post donation PLT ct reduced to < 100×109/L (range 80-100) in five donors (N = 5/237, Amicus). The pre donation PLT ct in them was 150-200×109/L. Collection efficiency (percent) of Amicus (79.3) was better as compared to the other two machines (CS: 62.5, Cobe: 57.5). PC collected on Cobe spectra had <1×106 WBC. The donor pre donation PLT levels had a positive correlation to the product PLT yield (r = 0.30, P = 0.000).Conclusion:Monitoring donor blood counts helps to avoid pheresis induced adverse events. A cautious approach is necessary in donors whose pre donation PLT ct is 150-200×109/L. The main variable in PLT yield is donor PLT ct (pre donation). High collection efficiency is a direct measure of an optimally functioning cell separator.

Platelet recovery in the recipient is influenced by the transfused dose of platelets, which in turn is dependent on the quality of single donor platelets (SDPs) in terms of platelet yield. Various donor factors such as predonation platelet count and Hemoglobin (Hb) concentration affect the platelet yield. A total of 61 plateletpheresis procedures performed on intermittent flow cell separator (MCS3p, Hemonetics) were evaluated for platelet yield. A relationship between predonation platelet count and Hb concentration with yield of platelets was studied using Pearson Correlation. The mean platelet yield was 2.9 +/- 0.64 x 10(11). While a direct relationship was observed between predonation platelet count and yield (r = 0.51, P < 0.001), no such correlation was noticed with donor Hb concentration (r = -0.05, P > 0.005). The yield was > or =3 x 10(11) in >80% of procedures when the predonation platelet count was > or =250 x 10(3)/mm. Optimization of platelet yield, which is influenced by predonation platelet count, is an emerging issue in blood transfusion services. However, further studies in this regard are needed using more advanced cell separators.
