Se describe la producción científica peruana que sobre enfermedad renal crónica (ERC) se ha escrito en la literatura médica mediante una revisión bibliográfica no sistemática hasta abril del 2016. Los resultados se dividieron en seis áreas temáticas: prevalencia de ERC, etiología de la ERC, diagnóstico y manejo, cobertura de la atención, costos de la atención y mortalidad. Se encontraron 39 artículos, pero solo 16 se utilizaron para el análisis. La prevalencia de la ERC en algunas regiones del país llega a 16%. La diabetes y la glomerulonefritis son las causas más frecuentes ERC en hemodiálisis (HD). En Lima y Callao, menos del 9% de pacientes diabéticos tienen albuminuria en su evaluación. El despistaje de ERC en pacientes hipertensos es costo efectivo. Menos del 50% de la población que requiere HD la está recibiendo. La mortalidad de población incidente en HD puede llegar hasta el 50% a los siete meses. La principal causa de muerte es cardiovascular. En conclusión, La ERC es una patología frecuente, con problemas en el diagnóstico y tratamiento temprano. Hay problemas de cobertura de HD y la mortalidad en HD es alta.

Background Laparoscopic peritoneal dialysis catheter placement has expanded indications, although a relative paucity of data exists about the best configuration for improving outcomes. The purpose of this study is to investigate the role of different catheter configurations and pelvic fixation on catheter and patient outcomes. Methods Retrospective chart review of patients undergoing peritoneal dialysis catheter implantation between January 1, 2013, and December 31, 2016. All procedures were conducted laparoscopically at a single center. Statistical analyses were conducted using Stata/SE 14.2. Results Buried catheter configuration was a statistically significant predictor of peritonitis compared to unburied configuration (P = 0.008). Buried catheter was exteriorized at 100 days (SD 107.8). A longer length of time to exteriorization significantly correlated with peritonitis, need for revision, and need for revision pelvic fixation (P < 0.05). Additionally, initial pelvic fixation was a significant predictor of revision (HR 3.94). Conclusions Peritoneal dialysis catheter placement via a laparoscopic approach can be successfully performed in a diverse patient mix with positive results. However, buried catheter configuration and prophylactic pelvic fixation should be carefully employed in select patients.

SUMMARY The aim of this study is to discuss the global costs attributed to chronic kidney disease (CKD) and its impact on healthcare systems of developing countries, such as Brazil. This is a systematic review based on data from PubMed/Medline, using the key words “costs” and “chronic kidney disease”, in January 2017. The search was also done in other databases, such as Scielo and Google Scholar, aiming to identify regional studies related to this subject, published in journal not indexed in PubMed. Only papers published from 2012 on were included. Studies on CKD costs and treatment modalities were prioritized. The search resulted in 392 articles, from which 291 were excluded because they were related to other aspects of CKD. From the 101 remaining articles, we have excluded the reviews, comments and study protocols. A total of 37 articles were included, all focusing on global costs related to CKD. Despite methods and analysis were diverse, the results of these studies were unanimous in alerting for the impact (financial and social) of CKD on health systems (public and private) and also on family and society. To massively invest in prevention and measures to slow CKD progression into its end-stages and, then, avoid the requirement for dialysis and transplant, can represent a huge, and not yet calculated, economy for patients and health systems all over the world.
