Heart rate fragmentation gives novel insights into non-autonomic mechanisms governing beat-to-beat control of the heart’s rhythm

Abstract: Objectives To demonstrate how heart rate fragmentation gives novel insights into non-autonomic mechanisms of beat-to-beat variability in cycle length, and predicts survival of cardiology clinic patients, over and above traditional clinical risk factors and measures of heart rate variability. Approach: We studied 2893 patients seen by cardiologists with clinical data including 24-hour Holter monitoring. Novel measures of heart rate fragmentation alongside canonical time and frequency domain measures of heart ra… Show more

“…These HRV features can potentially be used to define a threshold to aid in predicting seizures (Moridani and Farhadi, 2017). This was supported by a recent study of 238 temporal lobe seizures from 41 patients where HRV features, including decreased RR and pNN50, helped to identify pre-ictal state in 90% of patients and 41% of seizures (Billeci et al, 2018;Leal et al, 2021).…”

“…RR interval fragmentation analyses that attempt to quantify the erratic beating of the SAN by detecting small, aperiodic long-range fluctuations in RR intervals are an expansion of the aforementioned RR interval variability analyses [ 34 ]. RR interval fragmentation parameters assessed include percentage of inflection points (PIP), or points at which the first RR difference changes sign; the percentage of short RR segments (PSS); the inverse of the average RR segment length (IALS); and the percentage of alternating RR segments (PAS).…”

“…Increased heart rate fragmentation (e.g., PIP and W3) has been shown in individuals with coronary artery disease [ 16 ], as well as in other cardiovascular disturbances [ 17 ], but its mechanism is still under investigation. In those conditions it is postulated that both intrinsic molecular alterations in the sinus node pacemaker cells and derangements of their autonomic control may play a role [ 18 , 19 ]. The infection with T. cruzi causes damage to all structures of the heart through cell toxicity and inflammation, including the cardiac intrinsic autonomic innervation, the exciting-conducting system and the contractile fibers [ 11 ].…”
