Background: Rapid antigen-detecting tests (Ag-RDTs) for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) can transform pandemic control. Thus far, sensitivity (≤85%) of lateral-flow assays has limited scale-up. Conceivably, microfluidic immunofluorescence Ag-RDTs could increase sensitivity for SARS-CoV-2 detection. Materials and Methods: This multi-centre diagnostic accuracy study investigated performance of the microfluidic immunofluorescence LumiraDx™ assay, enrolling symptomatic and asymptomatic participants with suspected SARS-CoV-2 infection. Participants collected a supervised nasal mid-turbinate (NMT) self-swab for Ag-RDT testing, in addition to a professionally-collected nasopharyngeal (NP) swab for routine testing with reverse transcriptase polymerase chain reaction (RT-PCR). Results were compared to calculate sensitivity and specificity. Sub-analyses investigated the results by viral load, symptom presence and duration. An analytical study assessed exclusivity and limit-of-detection (LOD). In addition, we evaluated ease-of-use. Results: Study conduct was between November 2nd 2020 and January 21st 2021. 761 participants were enrolled, with 486 participants reporting symptoms on testing day. 120 out of 146 RT-PCR positive cases were detected positive by LumiraDx™, resulting in a sensitivity of 82.2% (95% CI: 75.2%-87.5%). Specificity was 99.3% (CI: 98.3-99.7%). Sensitivity was increased in individuals with viral load ≥ 7 log10 SARS-CoV2 RNA copies/ml (93.8%; CI: 86.2%-97.3%). Testing against common respiratory commensals and pathogens showed no cross-reactivity and LOD was estimated to be 2-56 PFU/mL. The ease-of-use-assessment was favourable for lower throughput settings. Conclusion: The LumiraDx™ assay showed excellent analytical sensitivity, exclusivity and clinical specificity with good clinical sensitivity using supervised NMT self-sampling.

Rapid detection of infection is essential for stopping the spread of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The Roche SD Biosensor rapid antigen test for SARS-CoV-2 was evaluated in a nonhospitalized symptomatic population. We rapid-tested a sample onsite and compared results with those from reverse transcription PCR and virus culture. We analyzed date of onset and symptoms using data from a clinical questionnaire. Overall test sensitivity was 84.9% (95% CI 79.1–89.4) and specificity was 99.5% (95% CI 98.7–99.8). Sensitivity increased to 95.8% (95% CI 90.5–98.2) for persons who sought care within 7 days of symptom onset. Test band intensity and time to result correlated strongly with viral load; thus, strong positive results could be read before the recommended time. Approximately 98% of all viable specimens with cycle threshold <30 were detected. Rapid antigen tests can detect symptomatic SARS-CoV-2 infections in the early phase of disease, thereby identifying the most infectious persons.

Hintergrund Zur Reduktion des Risikos einer SARS-CoV-2-Infektion gelten in Gesundheitseinrichtungen bundesweit besondere Hygieneregeln. Trotz des von der Politik vorgegebenen Ziels einer möglichst flächendeckenden Durchführung von Tests zur Detektion von a‑ bzw. präsymptomatischen SARS-CoV-2-Infektionen im stationären Bereich ist ein äquivalentes Screening mit Antigenschnelltests für die ambulante medizinische Versorgung bisher noch nicht etabliert. Die Akzeptanz der erforderlichen Hygienemaßnahmen sowie eines solchen Screenings mit den erforderlichen Wartezeiten und Unannehmlichkeiten ist bei den hiervon betroffenen Patienten nur unzureichend untersucht. Zielsetzung Es wurde eine standardisierte, anonyme Befragung von Ambulanzpatienten zur Akzeptanz der erforderlichen Hygienemaßnahmen sowie zur Teilnahme an Screeninguntersuchungen mittels Antigenschnelltests durchgeführt. Außerdem wurde die Bereitschaft zur Durchführung einer SARS-CoV-2-Impfung erfragt. Ergebnisse Es wurden 195 (37 %) der 534 erhaltenen Fragebögen von rheumatisch sowie 339 (63 %) von orthopädisch erkrankten Ambulanzpatienten im Zeitraum vom 07.–15.12.2020 ausgefüllt. Es zeigte sich jeweils eine hohe Akzeptanz für den vorgeschriebenen Mund-Nase-Schutz (475 von 534 Patienten [89 %]) und die Maßgabe, derzeit auf eine Begleitperson zur Einhaltung der Hygieneregeln in den Wartebereichen zu verzichten (450 Patienten [84 %]). Auch die Durchführung eines verpflichtenden, kostenlosen Antigenschnelltests und die damit in Kauf zu nehmende Wartezeit von 15–20 min vor dem Krankenhaus wurde von einer großen Mehrheit der Patienten (428 von 534 Patienten [80 %] bzw. 449 Patienten [84 %]) unterstützt. Es bestand bei über der Hälfte der Befragten eine Impfbereitschaft („ja sofort“ 137 [26 %], „ja vielleicht“ 142 [27 %] der 534 Patienten), wobei signifikant ( p < 0,05; Chi 2 -Statistik) mehr männliche, mehr rheumatisch erkrankte und mehr ältere Patienten (> 60 Jahre) einen Impfwunsch angaben. Schlussfolgerung Die Ergebnisse zeigen eine hohe Akzeptanz von COVID-19-Hygienemaßnahmen inklusive eines Eingangsscreenings mittels Antigenschnelltest bei Ambulanzpatienten.
