Hepatitis C virus (HCV) is a global problem and the genetic diversity in non-Western countries particularly in Egypt, where the highest prevalence of HCV is very important because of showing distribution of different HCV genotypes . This study aimed to report phylogenetic analysis of hepatitis C virus isolates based on the 5` untranslated region (5`UTR) in Egyptian populations using 51 samples with high and moderate viral load , selected from different regions in Egypt. Samples were identified based on RT-PCR amplification of the 5` UTR. The data were confirmed by sequencing and phylogenetic analysis using Multiple Sequence Alignment to describe the similarities of different isolates of HCV. The results appeared a high level of genetic diversity of genotype 4 with 90.2% that was commonly found with its subtypes ranged 4a (74.5%) ,4l (5.9%) ,4v (5.9%) and 4n (3.9%) while genotype 1 recorded 9.8% with its subtypes ranged 1a (3.9%) ,1b (2.0%) and 1g (3.9%). Using Multiple Sequence Alignment of HCV isolates indicated high percent of similarities between different genotypes of isolates. The results obtained in this study provide a strong evidence for high percent of similarities between different genotypes of isolates obtained from different Egyptian regions.

MicroRNA (miRNA), which has been shown to correlate with liver functions, has been proposed as a new biomarker reflecting liver injury. The aim of the study was to investigate miRNA-122 (miR-122) and mir-RNA-199a (miR-199a) as a biomarker for predicting therapeutic efficacy in hepatitis C (HepC) patients. A total of 47 HepC 1b patients and 16 healthy subjects were enrolled in the study. Serum and exosomal mir-RNAs and other conventional biomarkers reflecting liver function were evaluated. The miR-122 levels in serum (miR-122 ) and exosomes (miR-122 ) were significantly lower in the Hepatitis C virus (HCV) genotype 1b patients than in the normal controls, but these levels were higher compared to the non-genotype 1b group. The mean miR-122 level in the sustained virological response (SVR) group was significantly higher than that in the non-response (NR) group (P < 0.01), and the miR-122 level in the SVR group was also higher than that in the NR group (P > 0.05), although this difference was not significant. miR-199a levels showed similar trends with the miR-122 levels in serum and exosomes. HCV RNA was negatively correlated with the miR-122 (r = -0.473, P = 0.004) and miR-122 (r = -0.424, P = 0.009) levels. miR-122 levels were positively associated with miR-199a levels (r = 0.453, P = 0.002) Univariate and multivariate regression analyses reveal that the miR-122 levels and ALT/AST ratio demonstrated a predictive value in evaluating patient outcomes. Serum miR-122 and miR-199a are potential biomarkers that reflect therapeutic efficacy.

Background and Aim This single‐arm, open‐label, multicenter, phase 3 trial evaluated the efficacy and safety of seraprevir, an hepatitis C virus (HCV) nonstructural protein 3/4A (NS3/4A) inhibitor, combined with sofosbuvir for treating Chinese patients with chronic HCV infection without cirrhosis. Methods Treatment‐naive or interferon‐experienced adult patients without cirrhosis were treated with a universal, combinational regimen of seraprevir 100 mg, twice daily and sofosbuvir 400 mg, once daily, for 12 or 24 weeks. The primary efficacy endpoint was sustained virologic response at week 12 after treatment (SVR12). Results Overall, 205 patients with genotype 1 HCV infection without cirrhosis were enrolled from 23 sites, 202 of whom completed the full treatment and post‐treatment course and 3 discontinued follow‐up. In total, 27 patients (13.2%) were interferon experienced. SVR12 was achieved by 201 out of 205 (98.0% [95% CI, 95.1%, 99.5%]) patients, 100.0% of patients with genotype 1a, and 98.0% of genotype 1b. In the other exploratory study, SVR 12 was achieved by 100% patients with genotype 2 (n = 21), genotype 3 (n = 7), and genotype 6 (n = 8). The majority of adverse events were mild to moderate and transient and did not require a specific medical intervention. Conclusions The all‐oral, ribavirin‐free regimen of seraprevir and sofosbuvir is an effective and well‐tolerated treatment option for Chinese patients mono‐infected with HCV, including those with a history of interferon treatment.
