Hair Coil Penile Tourniquet Syndrome in an Unusual Age

Abstract: Penile tourniquet syndrome (PTS), a rare urologic emergency, may lead to undesirable results including necrosis and amputation of penis, if not diagnosed and treated appropriately. Sometimes these injuries may be accepted as a forensic case. Miscellaneous objects used for strangulation can be metallic or nonmetallic. Of all ages, the most vulnerable period is infancy. Telogen effluvium is the most common cause of PTS in infants who are 0–6 years old. In the literature, telogen effluvium as a reason of PTS was … Show more

“…Afterward, this hair starts to dry up with gradual tightening because of the dryness over the coronal sulcus, causing gradual chronic ischemia which causes a little discomfort to the child. Initially the hair-tie causes edema and glandular deterioration followed by superficial penile injuries, urethral fistula, and even complete urethral transaction as in our case [ 5 , 6 ].…”

“…The hair coil strangulation is the most common cause with 79% of incidences. The rings and bands are used in adults to improve erection in most of the cases and frequently described in the occident, while strangulation by hair is more common in children of mothers with postpartum excessive hair loss (telogen effluvium) [ 5 ].The human hair is caught at the ridge of the coronal sulcus. Afterward, this hair starts to dry up with gradual tightening because of the dryness over the coronal sulcus, causing gradual chronic ischemia which causes a little discomfort to the child.…”

“…If it goes unrecognized, the condition can lead to devastating complications [ 2 ]. It is easily preventable, and when recognized early, it can be treated simply by removal of the coil under local anesthesia [ 3 ].…”

“…When it involves the glans penis, it is referred to as penile hair tourniquet syndrome (PHTS) or hair coil penile strangulation [ 1 ]. The hair gets wrapped around the coronal sulcus of a circumcised child, leading to a spectrum of clinical situations ranging from mild swelling and redness of the glans to more serious complications such as urethro-cutaneous fistulas, gangrene, and penile amputation [ 2 ]. We are reporting this case because a delay in diagnosis—due to a lack of awareness among general practitioners, or uncareful examination—can lead to devastating complications [ 3 ].…”

“…Penile hair tourniquet syndrome (PHTS) is a rare condition characterized by a progressive hair coil penile strangulation mainly occurring in toddlers (3). This clinical situation is also known as "progressive penile strangulation, hair coil penile strangulation and penile tourniquet syndrome" (4).…”
