Haemodynamic instability and myocardial ischaemia during carotid endarterectomy: A comparison of propofol and isoflurane

Abstract: (Group Prop; n = 14) and the other isoflurane/alfentanil (Group [so; n = 13) . Periods of haemodynamic instability were correlated to episodes of myocardial ischaemia as assessed by Holter monitoring (begun the evening before surgery and ceasing the morning 9.t" the first postoperative day). In Group Prop, anaesthesia was induced with alfentanil 30 #g" kg -z iv, propofol up to L5 rag" kg -t and vecuronium 0.15 mg" kg -z, and maintained with infusions of propofol at 3-12 mg" kg -t" hr -1 and alfen-

“…The period of highest risk for myocardial ischaemia is during emergence [14,15] and haemodynamic changes need to be particularly well controlled during this period. In this study, MAP and HR increased significantly at extubation in the remifentanil group, which although sustained in recovery, was not in excess of baseline.…”

“…Mutch et al [ 23 ] reported that hemodynamic instability is associated with myocardial ischemia during CEA. In our patients, the MAP was maintained at 70–130% of the ward baseline value and heart rate at 50–100 bpm.…”

“…General anesthesia has its own obvious attendant risks. Hemodynamic instability necessitating the use of vasoactive drugs is a major disadvantage of general anesthesia [20]. There have been conflicting reports regarding the increased incidence of postoperative ICU admissions, increased hospital stay, and costs when general and regional anesthesia are compared [21,22].…”
