Haemodialysis in Poisoning by Sea-snake Venom

Abstract: SummaryMyoglobinuria, renal failure, and muscular weakness or paralysis were noted in two patients following sea-snake bite. A pathological study by biopsy showed renal tubular necrosis and myonecrosis. Both patients were treated by haemodialysis, and there was a remarkable improvement in the muscular symptoms. Recovery was complete in both cases. It is believed that muscular paralysis was the effect of hyperkalaemia and uraemia on the muscles sensitized by the toxic effect of the sea-snake venom. “…The interstitial lesions consisted of edema and cellular infil trations. The findings were in accord with those observed in the previous reports [1,2,4,8]. Although glomerulonephritis and nephrotic syndrome have been described in man [11,12], this has not been our experience [2], In these two cases the glomerular changes in general were not remarkable except for mild mesangial proliferation.…”

“…Glomerulonephritis and nephrotic synsdrome are occasionally observed [11,12]. Renal infarct has been described [13], In our previous reports acute tubular necrosis with renal failure has been shown in patients bitten by seasnakes and Russell's vipers [1,2]. Further renal histological study in two additional patients with immuno fluorescent techniques forms the basis of this presentation.…”

“…Muscular symptoms associated with seasnake bite are improved by hemodialysis. 39 Alkalization of urine by sodium bicarbonate, if performed early-as soon as dark urine is observed or the snake is known to be myotoxic or hemotoxic-can prevent AKI in patients with myoglobinuria or hemoglobinuria. 28 In animal studies, alkalization of the urine before envenomation by Mulga snake (Pseudechis australis) or Hardwick's seasnake (Lapemis hardwickii) prevented renal pathological changes and impairment of renal function.…”
