Further Observations of Renal Insufficiency in Snakebite

Abstract: Acute renal insufficiency was observed in two patients bitten by Russell’s vipers. Both patients had bleeding symptoms presumably due to disseminated intravascular coagulation. There was mild intravascular hemolysis. The serum β-1 C globulin was decreased in both cases. Renal biopsy showed tubular necrosis and arteritis. By immunofluorescent study deposition of β-1 C globulin was noted in the arterial lesion, the glomerular mesangium, and the arteriolar wall. Immunoglobulin and fibrin deposits could not be det… Show more

“…Acute tubular necrosis (ATN) has not been observed after higher doses of bothropic venoms in experimental models. 6,15,18 In contrast, ATN has been described in humans following snakebite by various species, 4,10,12,[19][20][21][22][23] including Bothrops spp. 3,4,9,11,24 In this study, a dose of 0.4 mg of B. moojeni venom/kg produced ATN and functional abnormalities that peaked 5 hr and 16 hr after envenomation.…”

“…The lesion could have been missed if the renal biopsy was superficial. Segmental thrombophlebitis of the arcuate vein and its tributaries has been reported in both Russell’s viper bite and green pit viper bite 9,43 . Deposition of C3 without immunoglobulins in the wall of necrotizing arteries is demonstrable.…”

“…The mechanisms acting within the body to induce AKI during envenomation are uncertain. The pathogenetic mechanisms underlying acute renal failure (ARF) may include renal vascular obstruction by disseminated intravascular coagulation (DIC), ischemia or hypoperfusion due to the fall in blood pressure, and some extent of intravascular hemolysis lead to pigment nephropathy ( Sitprija et al, 1974 ). This manifestation does not exclude the presence of proteolytic enzymes and vasoactive substances, which could promote, or even potentiate, the coagulant process in renal sites.…”
