Guidelines for direct radionuclide cystography in children

Abstract: These guidelines, which summarise the views of the Paediatric Committee of the European Association of Nuclear Medicine, provide a framework which may prove helpful to nuclear medicine teams in daily practice. They contain information on the indications, acquisition, processing and interpretation of direct radioisotope cystography in children. The guidelines should be taken in the context of "good practice" and any local/national rules which apply to nuclear medicine examinations.

“…It is equally reliable in diagnosing clinically significant reflux, with nearly 20-fold lower radiation burden. 15 Setbacks mainly derive from lack of high anatomic resolution: nonvisualization of the urethra (indispensable in male infants with suspected posterior urethral valves); restricted capability for precise VUR grading; and limitations in detecting very mild reflux (of uncertain clinical significance, though). VCU is therefore compulsory initial approach in males, but DRC may be preferred for screening girls and high-risk cases like asymptomatic siblings of index patients, as well as for following diagnosed reflux in both genders, essential for prolonged surveillance requiring repeated studies.…”

“…There is a linear relationship between radiation dose to the thyroid and probability of future thyroid neoplasia without any threshold dose (Ron et al, 1989) and even this low radiation to thyroid can be hazardous. In our opinion, due to this phenomenon, using other tracers with particulate nature such as Tc-99m sulfur colloid (as it is currently recommended for direct radionuclide cystography (Mandell et al, 1997;Fettich et al, 2003)) seems to obviate this problem and warrants further studies in this field.…”
