The objective of the present study was to compare fasting levels of free IGF-I in serum from patients with adult onset growth hormone deficiency (GHD) and from healthy volunteers, and to examine the effect of GH replacement therapy in GHD on serum free IGF-I. Free IGF-I was measured using separation of free IGF-I by ultrafiltration in serum samples from 42 healthy volunteers and 27 patients with GHD, in the latter before and after 1 year of treatment with GH (2 IU/m 2 ) (n = 13) or placebo (n = 14). Free IGF-I was significantly decreased in patients with GHD (700 Ϯ 100 ng/l (mean ϮS.E.M.), range 55-2618 ng/l) compared with controls (1010 Ϯ 70 ng/l, range 231-2431 ng/l; P = 0.0016). Total IGF-I was 85 Ϯ 10 mg/l (GHD) and 160 Ϯ 10 mg/l (controls) (P < 0.0001). The ratio of free over total IGF-I was increased in GHD to 0.85 Ϯ 0.08% compared with 0.66 Ϯ 0.05% in controls (P = 0.04). In both GHD and controls, free IGF-I correlated significantly (P < 0.05) with total IGF-I (GHD r = 0.78; controls r = 0.42), IGFBP-1 (GHD r = ¹0.67; controls r = ¹0.46) and the molar ratio of total IGF-I over IGFBP-3 (GHD r = 0.58; controls r = 0.62). After 1 year of GH treatment, free IGF-I was increased to 2780 Ϯ 320 ng/l (P = 0.003) and total IGF-I was increased to 270 Ϯ 30 mg/l (P = 0.006) both of which values were greater than those in healthy volunteers. There were no changes in free or total IGF-I in the placebo-treated group. In conclusion, levels of free IGF-I are decreased in GHD, but measurements of free IGF-I in a single, fasting serum sample do not offer a better separation of patients with GHD from individuals with normal GH status than can be achieved by measurement of total IGF-I. A AB BS S T TR RA AC CT T O Ob bj je ec ct ti iv ve e: : Insulin-like growth factor-I (IGF-I) and IGF binding protein-3 (IGFBP-3) levels are important markers in diagnosis of growth hormone (GH) related disorders. The normal levels of IGF-I and IGFBP-3 vary among different ethnic groups, and using the references derived from different populations may sometimes be misleading during diagnosis, treatment and follow-up. We examined the levels of IGF-I and IGFBP-3 in healthy adult Turkish population. M Ma at te er ri ia al l a an nd d M Me et th ho od ds s: : Eight hundred and thirty-three subjects (512 females, 321 males) were enrolled in the study. Serum IGF-I and IGFBP-3 levels were measured by immunoradiometric assay in all participants. The study population was divided into age groups (18-20, 21-23, 24-25, 26-30, 31-40, 41-50, >50 years of age) and gender groups (females and males separately in the population ≤ 30 years of age, combined in age groups over 30 years of age) according to the references defined by the kit manufacturer and the results were compared to the reference values provided by the manufacturer that represents a reference population. R Re es su ul lt ts s: : Serum IGF-I levels were statistically higher than the reference levels in all age groups of women ≤ 30 years of age (p< 0.05). In men, IGF-I levels were significantly higher (p< 0.05) only in 26-30 years age group. In gender-combined groups over 30 years of age, IGF-I levels were statistically higher than the reference levels (p< 0.05). Serum IGFBP-3 levels were significantly lower than the reference values in 24-25 years age group in both genders and in 18-20 years of age in males (p< 0.05). Serum IGFBP-3 levels were significantly higher in 26-30 years age group in males and in all gender-combined groups > 30 years of age (p< 0.05). Abstract.We studied the effects of short term replacement with human GH at three doses (0.124, 0.250 and 0.375 IU/kgBW/week) in 12 adult patients with GH deficiency (GHD). The patients were divided at random into three goups of 4 patients (groups A, B and C) and each group was treated with three doses of GH and placebo for 10 weeks in shifts of two weeks each. The replacement was started with one of three doses of GH given sc daily at 2100 h for 2 weeks, which was followed by placebo treatment for 2 weeks. The various doses of GH and placebo were then given alternately.GH treatment increased serum IGF-I and IGF-BP3 levels in all the patients examined although the responses were partly influenced by the order of GH treatment. When the data obtained with the same doses of GH in the three groups were combined, a dose-response was demonstrated.There was a close correlation (r=0.726) between serum IGF-I and IGFBP-3. Serum triiodothyronine as well as non-esterified fatty acid (NEFA) levels also increased after GH replacement.Adverse side effects included edema in two cases and sleep distress in one case during the GH treatment at the highest dose of 0.375 IU/kgBW/week. These findings indicate that short term replacement with GH at the doses of 0.125 and 0.250 IU/kgBW/ week is safe and effective in adult patients with GHD.

Abstract: Based primarily on IGF-I and IGFBP-3 levels our data suggest that a GH replacement dose in young GH deficient adults in the order of 1-2 IU/m2 per day is adequate. This is a relatively low dose as compared to dose regimens in children and adolescents.
