A systematic review and network meta-analysis (NMA) of randomized controlled trials (RCTs) evaluating the core components of cardiac rehabilitation (CR), nutritional counseling (NC), risk factor modification (RFM), psychosocial management (PM), patient education (PE), and exercise training (ET)) was undertaken. Published RCTs were identified from database inception dates to April 2017, and risk of bias assessed using Cochrane’s tool. Endpoints included mortality (all-cause and cardiovascular (CV)) and morbidity (fatal and non-fatal myocardial infarction (MI), coronary artery bypass surgery (CABG), percutaneous coronary intervention (PCI), and hospitalization (all-cause and CV)). Meta-regression models decomposed treatment effects into the main effects of core components, and two-way or all-way interactions between them. Ultimately, 148 RCTs (50,965 participants) were included. Main effects models were best fitting for mortality (e.g., for all-cause, specifically PM (hazard ratio HR = 0.68, 95% credible interval CrI = 0.54–0.85) and ET (HR = 0.75, 95% CrI = 0.60–0.92) components effective), MI (e.g., for all-cause, specifically PM (hazard ratio HR = 0.76, 95% credible interval CrI = 0.57–0.99), ET (HR = 0.75, 95% CrI = 0.56–0.99) and PE (HR = 0.68, 95% CrI = 0.47–0.99) components effective) and hospitalization (e.g., all-cause, PM (HR = 0.76, 95% CrI = 0.58–0.96) effective). For revascularization (including CABG and PCI individually), the full interaction model was best-fitting. Given that each component, individual or in combination, was associated with mortality and/or morbidity, recommendations for comprehensive CR are warranted.

Objetivos: Visualizar os impactos do isolamento social sobre a prática de atividades físicas e suas consequências para o público idoso assim como identificar possíveis alternativas para a prática durante o isolamento e formas de assistência para redução de malefícios. Metodologia:Trata-se de umarevisão sistemática onde foram utilizadas as bases de dados SciELO, Medline, Lilacs e PUBMED. Foram utilizados para composição do espaço amostral apenas trabalhos publicados a partir do ano de 2010. Após remoção das duplicações, aplicação dos critérios de inclusão e exclusão e leitura para descarte de trabalhos não relacionados à temática da pesquisa ficaram 15 artigos. Resultados: A análise dos trabalhos selecionados sugere que o isolamento social pode trazer malefícios como o aumento da probabilidade do aparecimento de doenças, especialmente as relacionadas ao sistema cardíaco e psicológico. Foram observadas na literatura possíveis ferramentas a serem utilizadas para combater os malefícios causados, como a orientação de atividades por conferência, o incentivo por parte de familiares e amigos para prática de atividades físicas e de lazer e o desenvolvimento de programas públicos voltados ao estímulo da socialização do público idoso. Conclusão: A reclusão social aumenta a possibilidade do surgimento ou agravamento de sintomas relacionados à depressão, ou cardiomiopatias. Neste sentido é importante que o governo implemente programas voltados para socialização, como projetos de visitas domiciliares e o incentivo da prática atividade física em domicilio, orientando os familiares a acompanhar os idosos na prática de exercícios e atividades de lazer.

Influence of health promoting lifestyle on health management intentions and behaviors among Chinese residents under the integrated healthcare system

Background Health promoting lifestyle is an important influencing factor of individual health status. This study aims to assess the health promoting lifestyle of community residents in China, and explore its association with their health management intention and behaviors during the integrated healthcare system reform. Methods A total of 666 residents were recruited from six county level hospitals and 12 community health centers from July to August 2019 in Zhejiang Province, China. Health promoting lifestyle was measured by the Chinese version Health Promoting Lifestyle Profile-II scale (HPLP-II). Results The average total score of HPLP-II among our sample was 130.02±23.19. Among the six domains, interpersonal relationship had the highest average score (2.68±0.50), and physical activity scored the lowest (2.21±0.59). Total score of HPLP-II scale was negatively associated with being male (β = -0.13, p<0.01; Ref: female), positively associated with being students (β = 0.15, p<0.01; Ref: self-employed), and positively associated with a monthly per capita income of more than 8000 RMB (β = 0.15, p<0.01; Ref: less than 3000 RMB). The domain scores of HPLP-II were significantly correlated with residents’ health management intention and their behavior on following doctors’ advice or not. Conclusions The health promoting lifestyles of community residents in China are at moderate levels. Improving residents’ healthy lifestyle levels might be helpful for changing their health management intentions or behaviors.
