Grading of dysplasia in Barrett's oesophagus: substantial interobserver variation between general and gastrointestinal pathologists

Abstract: There was considerable interobserver variability in the interpretation of ND or IND/LGD in BO between non-experts and experts, but also between expert pathologists. This suggests that less subjective markers are needed to determine the risk of developing AC in BO.

“…Th at being said, there is reasonable interobserver agreement among GI pathologists for the extremes of dysplasia, namely IM without dysplasia and HGD/EAC ( 109 ). Th ere is considerably more diffi culty in the interpretation of indefi nite for dysplasia and LGD ( 121 ). Th e importance of the confi rmation of the diagnosis of LGD comes from two recent studies from the Netherlands.…”

“…Moreover, the role of high-resolution imaging in improving overall diagnostic accuracy should be further investigated; VFI image analysis algorithms that combine features from high-resolution imaging modalities such as confocal microendscopy and high-resolution microendoscopy should be tested to determine how the addition of higher resolution imaging might improve the overall detection of neoplastic lesions. Finally, given the known variability in differentiating BE, IND, LGD, HGD, and EAC, 34,53,54 in a larger clinical study, consensus pathology review with multiple independent pathologists is required to guard against the variability in the gold standard.…”

“…However, determining the presence of LGD may be more difficult, with higher inter-observer variability (56). Therefore, current evidence recommends confirmation of histopathological diagnosis by a second pathologist with extensive experience in the interpretation of BE dysplasia (21).…”
