ZuSaMMeNfaSSuNG Hintergrund Die Coronavirus-Krankheit COVID-19 ist eine neue, sich rasch ausbreitende zoonotische Infektionskrankheit, die der Weltgesundheitsorganisation (WHO) erstmals am 31. Dezember 2019 gemeldet wurde. Da zurzeit keine wirksamen Medikamente oder Impfstoffe zur Behandlung oder Prävention von COVID-19 zur Verfügung stehen, rücken nicht-pharmakologische Public-Health-Maßnahmen zur Eindämmung der CO-VID-19 Pandemie stärker in den Fokus. Studienziele Ziel dieses Rapid Reviews war es, den Effekt von Quarantäne -alleine oder in Kombination mit weiteren Public-Health-Maßnahmen -zur Eindämmung von COVID-19 zu untersuchen. Methodik Der vorliegende Rapid Review wurde von der WHO in Auftrag gegeben. Um der Dringlichkeit der Anfrage gerecht zu werden, wurde die Methodik eines systematischen Reviews punktuell abgeändert. Die vorliegende Publikation umfasst die wichtigsten Aspekte des Rapid Reviews und wurde vom WHO Collaborating Centre an der Donau Universität Krems (Österreich) auf Deutsch übersetzt. Ergebnisse Insgesamt wurden 29 Studien eingeschlossen. Zehn Modellierungsstudien befassten sich direkt mit CO-VID-19, 15 weitere Modellierungsstudien und 4 Beobachtungsstudien lieferten indirekte Evidenz zu SARS und MERS. Die Studien zeichneten alle ein ähnliches Bild -einen Vorteil von Quarantäne. Beispielsweise schätzten Modellierungsstudien, dass durch Quarantäne von Personen, die Kontakt mit Infizierten hatten 44-81 % neuer Fälle und 31-63 % an Todesfällen verhindert werden können, im Vergleich zu keinen Maßnahmen. Zwei Studien zu SARS zeigten, dass Quarantäne effektiver war und weniger kostete, je früher sie startete. Alleinige Quarantäne von Personen, die Kontakt mit Infizierten hatten, würde aber wahrscheinlich nicht ausreichen, um den Ausbruch von COVID-19 einzudämmen. Die Kombination mit anderen Maßnahmen wie physische Distanzierung oder Schulschließungen zeigte größere Effekte als Quarantäne alleine. Bei der individuellen Quarantäne für Rückkehrende aus Risikogebieten fand der Review vergleichsweise geringe Effekte. Schlussfolgerung Die Vertrauenswürdigkeit der Evidenz ist niedrig bzw. sehr niedrig, da die einzigen Studien zu COVID-19 Dieses Dokument wurde zum persönlichen Gebrauch heruntergeladen. Toolkit of methodological resources to conduct systematic reviews

Background: Systematic reviews (SR) can be classified by type depending on the research question they are based on. This work identifies and describes the most relevant methodological resources to conduct high-quality reviews that answer health care questions regarding prevalence, prognosis, diagnostic accuracy and effects of interventions. Methods: Methodological resources have been identified from literature searches and consulting guidelines from institutions that develop SRs. The selected resources are organized by type of SR, and stage of development of the review (formulation of the research question, development of the protocol, literature search, risk of bias assessment, synthesis of findings, assessment of the quality of evidence, and report of SR results and conclusions). Results: Although the different types of SRs are developed following the same steps, each SR type requires specific methods, differing in characteristics and complexity. The extent of methodological development varies by type of SR, with more solid guidelines available for diagnostic accuracy and effects of interventions SRs. This methodological toolkit describes the most up-to-date risk of bias instruments: Quality in Prognostic Studies (QUIPS) tool and Prediction model study Risk Of Bias Assessment Tool (PROBAST) for prognostic SRs, Quality assessment of diagnostic accuracy studies tool (QUADAS-2) for diagnostic accuracy SRs, Cochrane risk of bias tool (ROB-2) and Risk of bias in non-randomised studies of interventions studies tool (ROBINS-I) for effects of interventions SRs, as well as the latest developments on the Grading of Recommendations Assessment, Development and Evaluation (GRADE) system. Conclusions: This structured compilation of the best methodological resources for each type of SR may prove to be a very useful tool for those researchers that wish to develop SRs or conduct methodological research works on SRs

Purpose. To inform recommendations by the Canadian Task Force on Preventive Health Care on screening in primary care for the prevention and early detection of cervical cancer by systematically reviewing evidence of: (a) effectiveness; (b) test accuracy; (c) individuals’ values and preferences, and (d) strategies aimed at improving screening rates.Methods. De novo reviews will be conducted to evaluate effectiveness and to assess values and preferences. For test accuracy and strategies to improve screening rates, we will integrate studies from existing systematic reviews with search updates to the present. Two Cochrane reviews will provide evidence of adverse pregnancy outcomes from the conservative management of cervical intraepithelial neoplasia. We will search Medline, Embase, and Cochrane Central (except for individuals’ values and preferences, where Medline, Scopus, and EconLit will be searched) via peer-reviewed search strategies, and the reference lists of included studies and reviews. We will search ClinicalTrials.gov and the World Health Organization International Clinical Trials Registry Platform for ongoing trials. Two reviewers will screen potentially eligible studies and agree on those to include. Data will be extracted by one reviewer with verification by another. Two reviewers will independently assess risk of bias and reach consensus. Where possible and suitable, we will pool studies via meta-analysis. We will compare accuracy data per outcome and per comparison using the Rutter and Gatsonis hierarchical summary receiver operating characteristic model and report relative sensitivities and specificities. Findings on values and preferences will be synthesized using a narrative synthesis approach and thematic analysis, depending on study designs. Two reviewers will appraise the certainty of evidence for all outcomes using GRADE (Grading of Recommendations Assessment, Development and Evaluation) and come to consensus.Discussion. The publication of guidance on screening in primary care for the prevention and early detection of cervical cancer by the Task Force in 2013 focused on cytology. Since 2013, new studies using human papillomavirus tests for cervical screening have been published that will improve our understanding of screening in primary care settings. This review will inform updated recommendations based on currently available studies and address key evidence gaps noted in our previous review.Systematic review registration: not registered.
