Goiter Size and Thyroid Function in an Endemic Goiter Area in Northern Italy *

Abstract: Thyroid function was studied in a large number of subjects residing in Varsi, a town in the province of Parma, Italy. In this area, endemic goiter associated with moderate iodine deficiency [59 +/- 3 (+/- SE) microgram iodine/g creatinine], as defined by WHO criteria, affects 65% of the population. Serum T4, T3, thyroglobulin (Tg), and TSH concentrations were measured by RIA in 1218 subjects. The TSH response to TRH was determined in 108 subjects selected randomly from the groups with different grades of goite… Show more

“…These results are in agreement with the previous observation that thyroid function is not significantly affected in mild/moderate iodine deficient areas 16,17 . Goiter by clinical examination and ultrasonography was observed in 35% and 34%, respectively, of all subjects.…”

“…The Tg secretion rate was correlated to the thyroid weight. This is in accordance with the findings of elevated Tg concentrations in the peripheral blood in these patients and with the correlation between the size of the goiter and the Tg concentration (17)(18)(19)(20).…”

“…Serum Tg concentrations are elevated in various thyroid disorders (17)(18)(19)(20). A significant rise in the serum Tg concentra¬ tion follows a fine needle aspiration biopsy of the thyroid gland (21).…”

“…These reduced TSH levels especially among the elderly are in contrast to increased TSH as a direct effect of iodine de®ciency as observed in severe iodine de®ciency (20). This is presumably due to autonomous functioning thyroid tissue, as this regional difference in serum TSH was only found in subjects with nodular glands, and it is in accordance with data from moderate iodine de®ciency in Italy, where decreasing levels of serum TSH were found with increasing degrees of goitre at a clinical examination (21). A decline in serum TSH with age corresponds to the ®nding of an increased prevalence of overt as well as sub-clinical hyperthyroidism among the elderly in moderate ID.…”
