GM‐CSF, IL‐5 and RANTES immunoreactivity and mRNA expression in chronic hyperplastic sinusitis with nasal polyposis (NP)

Abstract: We conclude that GM-CSF, IL-5 and RANTES are produced in increased amounts in both allergic and non-allergic NP. Distinguishing features of non-allergic NP include fewer numbers of CD3 T lymphocytes, fewer IL-5+/CD3+ T lymphocytes and greater numbers of IL-5+ eosinophils. These differences may suggest different mechanisms of eosinophil accumulation and activation in allergic vs non-allergic NP.

“…It has been speculated that nasal inflammation induced by IgE-mediated mechanisms favors the development of acute and/or chronic sinus disease. A similar inflammation is observed in the nose and sinuses of patients with allergic rhinitis (2090)(2091)(2092)(2093)(2094)(2095). Moreover, sinus involvement has been observed by CT scans in allergic patients during the ragweed pollen season (2096).…”

“…Fibroblasts can produce RANTES (3,4), eotaxin (54), monocyte chemoattractant protein-1, IL-8, GM-CSF, G-CSF, or TGF-␤ (55). RANTES proteins were highly detected in nasal polyps compared with control normal mucosa (7). Cultured nasal polyps release RANTES to attract and activate eosinophils (5,6,8).…”

“…9,13,14,15 Because eosinophils themselves are a prominent source of many of these mediators, CHES can best be thought of as a disease of unrestrained self-perpetuating inflammation and that once eosinophils are recruited, they provide the growth factors necessary for their further recruitment, proliferation, activation, and survival. 13,14,16,17 This certainly is consistent with the chronicity of this disorder and the requirements for frequent surgical revisions. As such, although allergy might be important in precipitating or possibly exacerbating the disorder, it has to be recognized that ongoing allergen exposure may not be required for its persistence.…”
