Glued periosteal grafts in the knee

Abstract: Periosteal grafting was performed in 4 patients with osteochondritis dissecans of the medial femoral condyle and 1 patient with osteonecrosis of the lateral femoral condyle following prednisone therapy. The lesions were drilled out deep into the cancellous bone. The periosteal graft was taken from the medial facet of the tibia and fixed to the excavated bony defect by the tissue glue Fibrinkleber Human Immuno (Tisseel). The patients were followed clinically, by arthroscopic examination and by radiography at 3,… Show more

“…There are only a few studies of repair enhancement of isolated chondral defects in patients (Engkvist and Johansson 1980, Sully et al 1980, Niederman et al 1985, Muckle and Minns 1989, Hoikka et al 1990, Homminga et al 1990, Jensen and Bach 1992, Bulstra et al 1993, Brittberg et al 1994a, 1994b, Lorentzon et al 1996 and usually the number of patients is small. Tippett (1996) and Johnson (1996) have reported the clinical outcome with use of Pridie drilling or abrasion chondroplasty with or without osteotomy for knee chondral defects.…”

“…Various techniques have been developed to repair articular defects, for example, articular resurfacing, 1,2 transplantation of heterologous or autologous chondrocytes, [3][4][5][6][7][8][9][10] periosteum, [11][12][13][14] perichondrium, [15][16][17] osteochondral grafts, 18,19 meniscal allograft, 20 and prostheses using artificial materials. 21 These have been studied in animals and humans by many researchers.…”

“…These characteristics make autologous periosteal transplantation a viable method for treating cartilage defects. This procedure is used clinically to treat patients [1,18,23,24,30,45], and is also used experimentally in animals [26,28,32,33,46,49]. The main limitation of this procedure is the variability in cartilage produced by the transplanted periosteum.…”
